thank you for your help. Wondering what’s next after xtandi and starting zytiga + prednisone

Hi!

Me again. I just wanted to thank everyone who commented on my last post about affordable aberitone. My father picked up his prescription this am and was able to advocate for himself a bit more with his new (temp) doctor based on some of the feed back y’all gave.

I was wondering what the next steps looked like from here from this community. Has anyone become or known anyone who became resistant to xtandi and the started aberitone + prednisone and what happens next. Chemo, I suppose if you want. Seems like heavy duty stuff! What does that look like and for how long?

My fathers doc reminded him that he was terminal today so I guess I’m trying to party plan all of the joyous things in a functional timeline while also ramping up support for my father in ways he might need but not tell us.

A recent clinical trial called CARD confirmed that switching anti-androgens is not as effective as going to chemotherapy, then trying another anti-androgen.

This was the case for me. I was treated successfully on Zytiga for over 4 years. When that failed, my doctor put me on Xtandi even though I didn't think it would work. I got nothing out of it, PSA continued to rise and progession on PSMA scans confirmed it wasn't working.

I've done 4 cycles of Cabazitaxel chemotherapy now and started Xtandi again. So far it seems to be working as judging by my significantly reduced pain level. I've been pain free since restarting Xtandi. When I first tried right after Zytiga, it did nothing.

More recently, the PRESIDE trial is showing that adding Docetaxel chemotherapy to Xtandi is increasing resistance to Xtandi. Alternating chemotherapy with anti-androgens or adding chemotherapy to anti-androgens is superior to switching between them.

As regards Prednisone, I have not had any negative reaction to it. Under advice of my MO, I sometimes take an extra one if the Zytiga takes me to low (which it sometimes does).
