F/U on my post about Lynparza on my hubby, suggesting biopsy prior to IMRT, but at 80 yrs old, worried about risks going through bone and side effects. Dx last year with pc with meths to bones, G9, Orch last year, been on Nubeqa, other mets stable and psa still at <0.1. New met in Sacrum RO says was very small on scan 2/22, but has grown since to pretty large. Anyone else had this kind of biopsy where the tumor is inside the bone? MO is willing to proceed with IMRT w/o biopsy for urgency, then start Lynparza 30 days later. 2nd opinion suggests biopsy then IMRT. Biopsy will not make a difference in treatment with the RO, but says it could possibly make a difference with other treatment?

Radiation (SABR/SBRT) is painless, and completed in 3 sessions or less. Ask yourself what is probabilty of PCa in Scarum. High right? My RO at Memorial asloan Kettering said of my T5(spine) their is risk for biopsy. You are here for 5 days of Prostate, 2 institutions hv told you this probably is PCa. Lets do this in 3 days. You've already had a Foundation One report, right? It showed he was BRCA+, which is why he will be getting Lynparza, right? IMRT to the sacrum may prevent complications later.
Yes, Foundation One report from liquid biopsy, blood, showed CDK12. Lynparza has been approved by the FDA for this mutation. Thanks TA

Have not, afraid at 80 yrs old it might be too harsh? Or if he was not in the Lynparza group, we might waste time? Idk really?
Seems reasonable. PARP inhibitors can be harsh, but good response with CDK12 to Zytiga+PARP inhibitors.
I was 82 and went through it all with not a lot of difficulty. I think doing it is better by far than not.

What specifically are you asking about the sacrum biopsy?
I’ve got mets in sacrum and ramus/acetabulum and am scheduled for ct-guided deep bone biopsy this Friday. I think they’re just doing the ramus but imagine the process is similar. Sedation and local anesthetic.
My RO was ready to start treatments until I mentioned the bone biopsy, then she said we’d wait for the results of that.
My MO was quite upset when she thought the RO was going to start before the biopsy. Not sure why, perhaps the radiation can mess with the biopsy results?
I am 78 and was on Lynparza for 4-5 months with no side effects. Work for a few month and then PSA starting rising again. MO to me off it.
