Dear friends .....Need advice starting prostate radiotherapy on oct 21 2019

After 3 months of Firmagon.... with PSA from 33 to 0.6 and Glieson 8....

Age 79 but no other health problem. No symptoms as of now....except I have T3B with 2 small dots on Ilias bone in Choline scan....

I am getting RT at Toulon France......

I am starting radiotherapy .... I dont know yet if it will be 2 Gray or 3 gray depending on Anatomy of prostate.... I will know by 18 oct....

I am anxious about organ damage during IMRT/IGRT plus Arc etc.....

I would like to have feedback on how to keep Rectum les nervous....

Bladder should be OK .... just dose with water....

For Rectum and Bowl..... lot of questions....

My RO thinks just stay cool.... eat low residu ... drink lot of water.....and he laughs

May I share your experience....on

1 how to keep rectum cross section in check

2 should I use Movicol ....during the treatment till I have some diarrhea...My RO does not recommend....

3 what to drink and eat so as to make good cells grow every day after damage from RT

Your RO is right- avoid taking drugs/fiber/laxatives that increase bowel motility. Bowel motility will increase the number of beams that miss their target and hit healthy tissue. Your RO will contour the beams and leave a small margin at the rectum - that will do more than anything you can do to spare your rectum. He has rectal dose constraints he follows. Rectal injury is not a major lasting side effect of primary radiation using modern technology.

I was thinking of taking Movical.....so no use

Is cranberry juice useful ? seems it may put a protective layer ?

No- do not take cranberry juice. It stimulates the kidneys and may make urinary symptoms worse. Just water - no caffeine, alcohol, or fruit juices. And restrict fluids at night. No supplements. Exercise!

If not....just water...

That’s a great drop in Psa . Fatigue as RT went on and afterwards became an issue for me . Walk even when you feel like sleeping . Get some oxygen flowing each day . My first shot of firmagon brought my Psa down to undetech along with 8 weeks imrt . The combo was effective for me and put me into remission with my first scans after treatment .. May you have this same luck . Good luck proceeding in healing yourself .

How low PSA may go with only Firmagon....I do not agree with my URO who says Firmagon should bring PSA to below 1....without RT.....

He thinks RT does not affect PSA.....Strange.....

After 3 months...with Firmagon my PSA is from 33 to 0.57...

I also started double adt prior to RT . This can work well for you as well.,( using adt first ) that firmagon is in my view the best ...It brought my Psa down with the first shot as well ... Hang in there Karmaji ...

You are doing so well and not giving the gentle methods the time needed to clean up these mets. Radiation is a sledgehammer (which has its place) but only in the West is it usual to not wait for things like ADT to do their work over a period of months so radiation becomes unnecessary. Sometimes it is an advantage to not have enough money to pay for "extras". Good luck to you on your journey.
