New results after 4 months ADT not good. Any advice ?

I restarted ADT in June after a rise of PSA to 18.9. After a few years off(read my bio) it seems like the criters are back after me. One month of firmigon and 3 months on Orgovyx and my test results came back today at PSA 27.98 and Testosterone 20. The T seems reasonable but my PSA should be dropping. Would anyone know what might be going on here? I have scheduled with the Uro and the Onc coming up in a few days so i would like to here any ideas from you folks going in. Should I add something to the regiment. I would like to be more familiar with treatments available now. I am currently in no pain , and really only have issues with hot flashes every 2 hrs at night.

It's called castration resistance. You have 3 options of medicines to add to Orgovyx: docetaxel, abiraterone or enzalutamide. You can also try Provenge. If you feel any bone pain, you can try Xofigo.

Thank you for the quick analyisis. I was afraid of that senario . I dont seem to have much bone pain just alot of general aches and pains. I will look into the various side effects associated with these drugs and maybe my Onc will have a suggestion as to the best course. Sounds like I need to add something pronto!
