Thank you for the responses on the last two Swollen legs posts in regards to my fathers health. I feel obligated to share the latest info. At least I can do since you gave me so much things to think about.
Finally yesterday we were able to see my fathers oncologist. Unfortunately, as I said on the previous post they don't pay attention to the patient that much. Issue with legs appears to be little less these days, or maybe my dad has bigger problem with pain in the back and leg so he is not that focused on the edema.
In short:
Oncologist concluded that Zytiga is no longer working and prescribed Docetaxel 75mg + prednisone 10mg every 21 days.
My dad is also taking rivaroxaban for high D-Dimmer. (original 1437 - after 6 days around 900 ) .
My questions at this moment would be: Since he had good response on Zytiga, almost 2 years. Before Zytiga PSA around 750 , lowest 46. Now around 80. Do you think Docetaxel will help him and will have good response.
Another more important question about his pain. He was using ibuprofen, sometime diclofenac and these two medications really gave him some pain free time.
Now oncologist said since he is on Rivaroxaban he should be using Tramadol 4 times a day 50 mg. The drug makes him sleepy and not functioning at all and he only had been taken it 3 times. Plus according to him even tho he is sleepy and sleepyng he can still feel the pain little bit.
What pain medication is helpfull in methastasic pain on the bones? Does anyone have experience with that?
Can ibandronat help with bone pain ?

In general when a second generation anti androgen fails, chemo will help to control the cancer and after the chemo the cancer may respond to second generations anti androgens again.
If he is treated with enzalutamide instead of chemo , in general enzalutamide or apalutamide may work for a short time since there is cross resistant among these new anti androgens.
The chemo may treat cancer in the bone and help to control the pain in the back, if the pain is caused by a metastasis. He could also request palliative radiation to the bone met which may be causing the pain. The best way to control the pain is to see is it is possible to treat the cancer.
He should be in bone agents such as Denosumab or zoledronic acid, to try tp reduce the incidence of bone events, such as fractures compressions etc.
Since the cancer is castration resistant you could request treatment with Provenge a vaccine which has shown to prolong life. It can be done along with the chemo.
Thank you for the info very much. Unfortunately Provenge is no longer available in Serbia as it appears to me. But i am looking forward and hoping that docetaxel will help. They prescribed paliative radiation as well but after docetaxel if it doesnt help with pain.
Hello,In my case, Zytiga also stopped work after 1.5 year, and my MO prescribed me docetaxel infusions. At this point, I had multiple bone metastases with PSA=6.6. Now I got 7 infusions, and my PSA decreased to 2.1. Unfortunately, the number of metastases was not decreased drastically. Hope after 10 infusions, results will be better.
My MO prescribed me hydrocodone/acetaminophen 10-325 as a pain killer.
