My husband has not responded to anything thus far . Zytiga, Lynparza, Keytruda. With the Docetaxel his psa just keeps going up. He just had his fourth round. Spoke with his local oncologist yesterday. He said if someone doesn’t respond to Docetaxel it ususally is a poor prognosis. I have not yet heard from Dana Farber Dr Beltran. I want to see if she feels the same. I want to know where we go from here. I very concerned to say the least. I’m going to ask her about Jentava in conjunction with another chemo . Sorry not sure if that’s the spelling. Also was told about BAT. Just really mind boggling why he hasn’t responded to anything. It makes me have to wonder. Something has to work. I’m anxious to hear from Dr Beltran. Please keep us in your prayers that something will work!!!

I would go for BAT first - then Jevtana plus Carboplatinum - hopefully LU-177 will be approved in the near future (a very viable option). My two cents.

BAT would be crazy. It is NEVER used in symptomatic men - it may accelerate progression.

You are in the right track trying to get the opinion of Dr. Beltran. You could request a biopsy (direct or a liquid biopsy) to do IHC studies to see if the cancer has evolved to NEPC.. This type of cancer may respond to chemo with Cabazitaxel and platinum compounds. There are multiple clinical trials for NEPC

Why your dumb MO is not ordering neuroendocrine biomarkers ? Dr Beltran can not give opinion without these biomarkers . What a waste of time..MO should order biomarkers of NE variant immediately.According to adaptive theory, If you continuously and very strongly suppress Androgen sensitive cancer cells that can lead to uncontrolled growth of other type of cancer cells like NE type. More you suppress...more rise in more aggressive cancer cells. These markers will help Dr Beltran to understand his case better.

My husbands local oncologist is not aggressive at all. Now Dr Beltran is Brilliant. I certainly will bring this up. I had no idea about this. Thank you so much.

I was in the same condition and my psa went up to 736. Then they put me on jevtana and carboplatin. Now my psa is down to 424 and dropping. Hope this works for you.

I strongly suggest trying Estrogen patches.

No guarantees but has worked for me in reducing PSA from 12.6 three years ago to <.008 and now remains steady in technical remission.

With a rising PSA I had radical prostatectomy and radiation prior to going to Estrogen patches.

There is a lot of clinical literature on this therapy, there are virtually no adverse impacts.
