Husband has stage 4 PC with Mets throughout his skeleton. Skull to femurs solid. He has done chemo, immunotherapy and on ADT. PSA is rising, last month at 80. We are looking into the LU 177 treatment. I’m wondering if anyone in similar situation has tried this and did you get positive results?

Xofigo is radium. One injection every 4 to 5 weeks for 6 months. Radium mimics the cancer in the bone. We are scanning next week to see if its working.
If you have not seen them before, here are some results involving large numbers of men under the controlled and well-documented conditions of the VISION clinical trial. Overall median survival improved from 11.3 months to 15.3 months. Radiographic progression times were shorter, but improved from 3.4 to 8.7 months.
Other replies you get here will of course be more anecdotal, but often useful in terms of many practical details of how individuals navigated their ways to their treatments, etc.
Lu 177 PSMA treatment has shown a survival advantage in patients with advanced PC and multiple previous therapies. Search here or in Google for the Vision trial. Dr. Hofman in Australia has shown it may be more effective than chemo with jevtana. This treatment witl treat bone, soft tissue and nodes mets.
Having said that, if the patients has extensive infiltration of the bone marrow by the cancer the treatment may not be so effective and it may have significant side effects . mainly bone marrow suppression due to the radiation coming from the Lu 177 PSMA. A Ga 68 PSMA PET/CT or a Pylarify PET/CT it will show if there is diffuse bone marrow infiltration.
If most of the cancer or a lot of the cancer does not express PSMA which can be determine by a 18 F FDG PET/CT and a PSMA PET/CT, then the treatment may not be effective.
In the case of diffuse bone marrow infiltration treatment with Ac 225 PSMA and possible Thorium may be more indicated than Lu 177 PSMA treatment. There are clinical trials for these ligands:
I have decided to have the PSMA-based Treatment in India, but there are 30 or more places is Germany, but still not FDA approved in the USA. At least one Clinical Trial in the USA might take him if they are not already full. I am going to New Delhi, India very soon.
I had the LU-177 treatment with Prof. Baum in Wiesbaden, Germany. I did not have the extensive metastases as you, but it zapped all of my mets and brought my PSA down to normal. I met patients while I was there that did have your degree of involvement and, after 2-3 treatments, were nearly met free. I was told the major decision maker is one's liver and kidney functions. If you are considering this, try to get an evaluation quickly as space is very limited and many, many men are applying worldwide. Highly recommend. Best of luck to you.

Not a bother at all. My PSA was 60, Gleason 9, soft tissue mets, no detectable bone mets. The treatment is very simple, just a single injection. I had no side effects and several other people I know also had no side effects. However, everyone is different and after getting the PSMA PET CT, you will be advised as to your situation. Let me know if you need any other information.
Not sure where you are located but I found a consultation with Luke Nordquist in Omaha to be of value in terms of discussing options. I ultimately went to Heidelberg for Lutetium and strongly recommend looking at their published data. Another helpful resource is to talk to Nat Lenzo in Australia via theragnostics. Not a big fan of the the theragnostics approach to capitalism but Lenzo is a good source of information and very experienced.

As tango said in his comment about it not working well for some people. You might want to read a little bit of my profile and some of my later post. I have had the LU177 (April of this year)and had no luck with it and it seems to have destroyed my hemoglobin and platelets another blood numbers . What you see on my profile Might help you with some of the decisions.
I had just done my third infusion of LU 177. After my second infusion, the PSA dropped from 4.08 to 0.814. Will be doing my blood test next week. Will share with the team the outcome. Side effects are definitely milder than Chemo. God bless
Everything is in the normal limits. PSA is 5.8. I still have my prostate. I highly recommend that you consult with people like Prof. Baum in Wiesbaden Germany, or LUM in Munich, or Heidelberg. These clinics have been doing it for a long time and know what to do. Clinical trials may be free, but.... Vienna Austria has several highly rated clinics as well and their radiation laws are not as strict as Germany's. Germany requires that you spend three days in the hospital after treatment, even though it has been proven that there is no risk.
