Slight PSA Rise Immediately (two weeks) After HDR Brachytherapy

I have high volume G-9 PCA with cribiform and intraducatal pathology. Started ADT plus Zytiga two months prior to HDR Brachytherapy.

PSA went from 10.53 to 0.7 to 0.35. I had HDR Brachy on 26-Jul-2021. Labs on 11-Aug-2021 all WNL but PSA bumped up to 0.45. Has anyone else seen a small increase in PSA right after HDR Brachy. I start EBRT on 23-Aug-2021 and I am still on the ADT plus Zytiga. Not really freaking out but disappointed there was not a decrease in PSA. Thank you for your thoughts.

What a-hole scheduled a PSA test 2 weeks after brachytherapy? Your first test should not be done for 3 months!

Thanks Tall_Allen, I think it was standing order for my monthly labs for the ADT and Zytiga. I thought they were going to hold the PSA, but someone didn't get the memo.

I think HDR Brachy might raise PSA temporarily.

As noted, way too soon and scary that they would do that (test so quickly or often). Besides, it should have been explained that with your gland still there, you will get PSA bouncing around, sometimes for quite a while. You'll need a few tests with a decent consistent reading to establish your nadir, and then you can assess the success of your treatment.

Thank you, I'll keep you posted. I am not going stress until I see what happens 6 months after the EBRT.

I had low dose Brachy back in mid 2010 and my PSA moved around in tiny increments but remains at Zero and a few points. Latest PSA was 0.2 if I remember correctly. My Onco Doc indicated that as long as we have a Prostate, there will never be a total Zero reading.

I'm currently going through a similar protocol, Orgovycx, LD Bachy and finishing EBRT this week at Huntsman. They don't do a PSA until 6 months post radiation.
