PSA Starting to Rise.

After 2 years of Firmagon/bicalutamide monthly and 1 year of PSA below 0.01 (lowest they measure in St. Louis) I went off in April. PSA rose to 0.01 in June and is 0.04 now. Should I worry? History: PSA 172 (10/17); Surgery (05/18); Radiation (spring 2019); Chemo (Fall 2019). Oncologist said undetectable under 0.1; won't do any scans until 1.5. Age 59. Not ADT resistant, oncologist thought it would be good to go off it for a while. Testosterone is up to a whopping 31!

There's no point to a vacation if you don't let it last long enough so that your T returns to normal and you feel better.

My History

Gleason 9

Nothing in the bone

Local Lymph nodes only

Did 25 session of radiation

Did high dose brachytherapy

Been on Fermigon for 2 1/2 years

Considering a vacation now

TA Would u take a vacation if you were me or stay on firmagon?

You are in a very different situation. It wouldn't be a vacation for you, it would be the end of ADT.

You have received whole pelvic RT and long-term adjuvant ADT, right? Hopefully, that will cure you and no further treatment will be required.

It takes a long time for testosterone to fully recover after Firmagon, it takes about a year. If you want a vacation I would drop Firmagon and continue with 150 mg Bicalutamide. The testosterone level will recover while you take Bicalutamide. Then continue with Firmagon when the PSA value rises to e.g. 2.0 ng/ml.

Those PSA numbers are still low. I would not worry about it for now. If he get 3 significant rises in row, then He might need to switch medications. For now stay the course.

I dont know if this is against any rules here but if you have rising PSA, why would you not, as an addition to whatever else you do, take sulforaphane per this fairly recent RCT? The actual product they used is available on Amazon (BROQ). No downside that I can see. cancerpreventionresearch.aa...

Celebrate. You are <0.1, undetectable. BTW, I stayed with ADT for six years until my MO talked me out of the injections. And a year later when T did not recover, to supplement testosterone with Androgel. That was eleven years ago. Still undetectable and T at 500 or so (with a low dose of Androgel.

Old Choppy, I recomment you go on Casodex monotherapy as recommend by GP24 and ride it as long as it aslong as you can. I am going on 3 1/2 years with a Psa now at 0.1 after dropping to 0.015 at one year. I expect to ride it ANOTHER 1 1/2 TO 2 YEARS AND THEN STOP and hope for cure with casodex withdrawal
