After 18 months olaparib it is finally getting resistant PSA going up from 5.5 to 8.8. I am brica2. My next step is the possibility of a Keytruda clinic trial. My lymph node Mets are still fairly stable but my bone Mets have started to increase.l would appreciate any info or experience anyone has on Keytruda or any other experiences with other approaches.

Docetaxel+carboplatin usually works when parp inhibitors work.
Also BRCA2+, and was on Lynparza for 2 years when it ran its course for me as well. Next went on to ARV-110 trial which failed me at the 8 week mark. Now waiting for triple therapy including Keytruda + Actinium + Xtandi at Weill Cornell, but delayed due to global shortage of Actinium. I won't be able to wait too long before I go on to Plan B. Expect combo chemo of Carboplatin and Cabazitaxl, as I have not had any chemo to date.
After my failure of Lynparza, I have not found any consensus for a definitive next step viable treatment for us BRCA2+ carriers. Although, Carboplatin could be effective.
If you get a good response with Keytruda and your PSA drops to undetectable, be alert for other changes that can be life threatening if left undiagnosed. Once the immune system is kick started by Keytruda, it has trouble stopping after all cancer and suspicious lesions are destroyed. Our cancer clinic caught the adrenal crisis and gave him a four month holiday to establish replacement levels of hydrocortisone and levothyroxine before continuing with the two year trial.
