Five months into ADT, severe muscle pain has emerged. Since I have not had any pain for decades except from bicycle falls, I assume the pain is cancer related. Since the area of pain has moved once, from just beneath my shoulder on the back side to my upper arm, both on the left side, I assume it is not the cancer per se but a side-effect of the ADT. I have four suspected but not confirmed mets in the bones.
I am using Firmagon/degarelix. In one study, albeit a bit dated it´s from 2011 havn´t found anything newer, Leuprolide is found to give less body pain than Firmagon with a p-value of just 0.006.
However, I am very hesitant in switching to leuprolide since Firmagon appears to have other significant long-term benefits which is why I chose this therapy. If what is said e.g. in this study still holds, I will stay with Firmagon if the pain can be kept tolerable i.e. not preventing me from sleeping.
I have used pain medication with limited success. Paracetamol tablet and Voltaren gel (diklofenakdiettylamin) on the area of pain.
Any comments would be appreciated.

Had the same problem, pain moving around around so much I thought I would have to play Whac-A-Mole. It was finally determined the cause was a cervical nerve acting up. One day it was gone.
When I was on Lupron and Zytiga I experienced weird random pains in my knees and shins. They would come and go and when I went off the meds the pains went away. One thing I have noticed both times while on Lurpon (the 2nd iteration is Lupron-only without Zytiga) is that my legs feel heavy. I run and it's just a chore to go out now. My legs feel like lead. It's very disconcerting.
I had similar aches and pains, what works for me is exercise. Walking and weight bearing exercises I do in the gym give me relief but I have to do it regularly. If not, the muscle and joint pains return, it is the new norm.
Coincidentally or not, the pain increased after intense weight bearing exercises yesterday. But of course I will continue my intensive exercise regimen, will be there today again.Right now the pain level is low, it´s so random.
The heavy leg syndrome is what i get....in as much as climbing stairs is a real chore....i do have spinal comp. And maybe that is a factor....but it seems to me that the neurological bs associated with luoron/erleada...may be adding to the mix ......GREATLY......AND NO all the jack lalanes out there exercise doesnt fix it....b.w.
Pain from a systemic medication would not be expected to be so specifically localized. You need to get an MRI of your cervical spine with special attention to the C-7 level ( and one lower and higher) and also visualizing the shoulder and scapula on the left). Would be most concerned for a met affecting the C-7 nerve.
I appreciate getting this angle. The pain becomes much worse after intensive weight bearing exercises, which has never been a problem before. If you have any idea whether this could be an indication for a bone met, please let me know.
A little here and a little there.... then a little there and a little here. Left foot then right foot.... right foot then left foot. Right hip then left hip. Left hip then right hip... I never knew I had so many places to ache....
I ask my par On a daily basis how are you? His reply will be well yesterday I had a good day as I know as I see him every day but today I’m not as good, my par can not distinguish his pain as it continues to move around constantly, and his feet he really struggles with, so just the same left side right side, here there, back ribs legs knees, how ever his mo his doing a mri scan to see if the mets wide spread in his back are adding to his pains hope your all well and keep smiling xx
