Hi All,
DX 2015 I have been on Lupron since 12/20 I went for next injection and psa doubled.4 to 1.10. I also took bicalutamide for a week before starting Lupron. Wondering if after 6 years if my cancer is becoming CR?

You have a Gleason 9. You may become castration-resistant. You could add abiraterone now.
Why does gleason matter in that scenario? Curious since my father is also G9.
I think with a Gleason 9 you usually have a higher tumor burden than with a Gleason 7. A higher tumor burden has more cells which can mutate to resistant cells. Therefore with a Gleason 9 you usually have a shorter time to castration-resistance than with a lower Gleason score.
These studies report the Gleason score as one factor, among other factors. However, they report a shorter time to castration-resistance than six years.

Would that be Xtandi?
You can add Xtandi or Zytiga (Abiraterone), there is not much difference. Zytiga is usually added and it is cheaper.
Xtandi is closer to casodex than zytiga.
But you have to continue with Lupron while taking Xtandi. Therefore I do not think there is a noticable difference in side effects between Xtandi and Zytiga.
As far as I know you don't have to take Lupron while on xtandi, I didn't. But then again I had the orchiectomy.
Xtandi was approved in combination with Lupron. If you had Orchiectomy, you do not need Lupron of cause.
When I have to choose between Zytiga and Xtandi, I would prefer to take Xtandi without Lupron but adding Tamoxifen. But this is not approved and I doubt if my doctor will accept Xtandi monotherapy.
One thought about Xtandi, if you can afford it or get help which is possible though grants ect, ,,Lu-177 on the horizon here and available in other countries works well with Xtandi . Just a thought. I have not seen data like this on Zytiga. Blue Skies

Another aspect of note is that with Xtandi you can avoid the steroid. It's well documented that steroids can have side effects some people find intolerable. Some people feel intense, jittery and achey. Xtandi is expensive though and there's no generic...yet.
