My father was diagnosed with grade 4 (Gleason 4 + 4) prostate cancer on 25/03/20. psa: 39 biopsy 12/7
There were many bone and lymph metastases in the 10/04/20 psma pet. 14/04/20 bilateral acetabulum and pubic bone 30 Gy 10 dose radiotherapy was applied and Eligard was started every 3 months.
24/10/20 psma pet: right lung 25mm (suvmax 4.31), right iliac (suvmax 14.64) and right 5 loste new lesions
02/11/20 Radiotherapy and xtandi started to 30 Gy 10 dose pelvic bones
10/02/21 psma pet
right lung (4.3 * 2.8) suvmax 6.2, prostate gland suvmax 3.2, right posterior suvmax 13, L4 vertebra suvmax 4

My questions
1-Should my father have had chemotherapy at the beginning or was he started xtandi?
2- For the lung, the 1st oncologist asked for biopsy, the 2nd oncologist said chemotherapy, the 3rd oncologist was xtandi, which one was correct?
3-Crp is climbing rapidly and the doctor did not say anything about it.
4-Are meat, milk and eggs bad for prostate cancer?
5-Is glutamine supplements harmful?
6-How do we know that we have neuroendocrine cancer?
my father will receive 5 doses of radiotherapy to his lung and chemotherapy will be started.

I think you should not be concerned with the past. The only thing that matters are the decisions you and your father make now.
Docetaxel is undoubtedly a good idea, as long as he doesn't have any problems that may stop him from getting chemo. He is starting to fail Xtandi, so now would be a good time.
Crp is just telling you that there is inflammation somewhere.
There is no credible evidence that meat, milk, or eggs are bad for prostate cancer. All the "evidence" is from observational studies that should not be used to change his diet.
Who told you that he has neuroendocrine cancer? That's very hard to believe, given his initially high PSA and excellent reaction to hormone therapy. It is diagnosed by a biopsy of one of his metastases. They would not be using docetaxel if they believed that.
Thank you very much for your comment. As you said, eligard and xtandi got good results, but still new metastases occurred, lymph nodes and some lesions in the bone got very small.He will take Radiosurgery for the lungs and dosataksel 50 mg every two weeks

I’m sorry about your father . Thank God for the loving child .. I too was 4+4 ,in 2015 .. with Imrt and double adt I’ve gone undetech for over five years now.. I wish the same for him ..#4 . I went on an all out holistic no animal protein or dairy diet on the onset . Many here don’t believe in this ... I Do! This is all an assault on our total being . Love is the best medicine . Cheer his spirit us any way you can . Good job advocating for him . 🙏❤️
