Started cabazitaxcel with carboplatin. Psa was at 78. First infusion was Monday. Looking at not so good news on mean survival rates. So what have we seen on trials? My MO is working all channels. So far just lymph glands, no liver or lung invasion.

You can read my summary in my profile. I have Small Cell cancer in the lining of my left lung. I responded well to Carboplatin and Etoposide 3 years ago. Currently on Lupron and Enzalutamide. Originally thought it was all Small Cell but now realize it is a hybrid with some original adenocarcinoma likely producing PSA. Happy to be alive and reporting. Prostate cancer for 25 years. Mets for 12.
Don't just look at trials, also look at off-label treatments. Small cell prostate cancer is similar to small cell lung cancer, so many of the approved treatments for SCLC will work for NEPCa.
FWIW, I'm not quite 14 months since my NEPCa diagnosis, and on my third chemotherapy, folfiri (a decades old colon cancer treatment recommended by my local oncologist). My recent scan showed fewer and smaller tumors, and my energy levels are improving.
Other options that have been discussed for me include PARP inhibitors, Opdivo + Yervoy, mTor inhibitors, other SCLC treatments, and of course, clinical trials. The problem I've had with clinical trials is that when I've needed a new treatment, there haven't been any trials that are a better match for me than an existing treatment. I've had my eye on the DLL3 trial that Dr. Beltran is involved with and Tall Allen is a big fan of, but if folfiri continues to work it would take a huge leap of faith to stop a treatment that's working and go into a clinical trial.
In other words, there's no shortage of treatments that can be tried, the trick is to stay healthy enough (particularly blood counts) to receive the treatments. I suspect I've already exceeded the median survival statistic and I'm still going and actually improving at the moment.
