Misha Beltran at Dana Farber is widely considered a world expert, but other experts include Armstrong at Duke, Sartor at Tulane, and Aggarwal at UCSF.
Small cell neuroendocrine cancer is very aggressive.
You really need to pick up the pace of your decision making.
I would immediately start seeking out some treatment second opinions from the experts mentioned by TA.
I had it diagnosed over 4 years ago. It was mostly in the pleura of my left lung. My story is in my profile. We were initially fooled by the pathology and the truth is that I have a mixed tumor consisting of the original adenocarcinoma and the small cell tumor line which morphed out of the adenocarcinoma after 25 years of treatments. I did respond to Carboplatin and Etoposide. Tumor genetics are essential.

He was diagnosed in March of 2020 with stage 4, spread to bones and lymph nodes Gleason score 10. He was started on hormone blocker, and chemo docetaxel, he had about 11 of those which made him really weak. He also went on Xandti for a few months which saw his psa drop to around 2. But it started rising and he started having alot of back pain, ended up in the hospital having to get a stent due to a blockage around the kidney which was causing urine to back up into the kidney. During the scan they saw mets to the liver, did a biopsy and found small cell . Since then he has had 2 sessions of platinum based chemo, one with some immunotherapy. The liver mets shrank and he seemed to be feeling better but now his stomach is swelling and he has alot of pain. More scans next week
Thank you for your detailed response. Sorry to hear that he's going through a lot of crap. As far as stents (for the kidney) are concerned, I've had many of them with no after pain or damage. Just make sure doc gives him an antibiotic every time the doc inserts or removes one.
It's a good idea to copy and paste your detailed post on his home page for future reference.

Yes and liver, lymph nodes and peritoneal spread
I've read about it. We saw his Dr today and he talked about what a fast moving cancer it is. We're trying to find the right pain meds to help. Pain is a constant issue
