Was hoping for a better monthly PSA post Xtandi

Hey gang,

PSA dropped to 8.9 from 9.2 last month with a month of Xtandi 😢

Started Xtandi after a jump from 5.2 to 9.2 in a month.

Was hoping for a bigger drop, it may be the dying PCa cells releasing PSA.

Have two 1 cm pelvic lymph node that had a high activity with Axumin scan a couple months ago.

Life is still good even with #stageIVpca

Still able to do dentistry(seeing patients are an antidepressant) , hiking, and made a full blown Friendsgiving dinner last Sat

Hope you have a nice Thanksgiving too brother Randy . You’ve still got lots of fire in that furnace buddy, probably that Xtandi will kick in harder in a bit ... it took me 90 days to hit nadir on it.

You hit it, dying cells are probably giving off PSA.

I'm 78. Give the Xtandi just a bit more time.MY experience is 2018 PSA 1300+.

Lupron till 7mos ago brought PSA below 2. 7mos ago PSA spiked up above 9.

Xtandi added for 6mos brought PSA down to .3 and still falling maybe.

Started Xtandi after SBRT to an enlarged lymph node so cannot cannot comment on PSA. Have you considered SBRT (focused radiation) on the lymph node. I saw a urological surgeon who wouldn't consider surgery. Too dangerous in my case. Been on Xtandi for 6-1/2 years now. Keep on keeping on. Sounds like you are doing all of the right things. Spoke to Kaiser MO and we are going to monitor monthly PSAs. If no significant drop, then a rechallenge with Taxotere in 2021.We ruled out radiation thus far, I'd rather for whole body systemic approach to get any stray PCa cells e.g. Chemo.

Chemo and lupron brought me from 59.9 to 7 right before the last chemo. It was over a year before Xtandi and Lupron brought me to <0.1.
