PSA level has increased since Xtandi

My husband was prescribed Xtandi in July 2020. His PSA levels fluctuated between a high of 9.1 and a low of 5.4. His latest PSA was 40. We will discuss this at next week’s oncologist appointment as well as where we go from here. Has anyone else experienced such a large increase with Xtandi and if so what was prescribed next.

Or try a clinical trial. Has he had any metastases biopsied?

You might request one - the newest and largest metastases are best. They can do cell histology, IHC, and genomics, depending on the amount of tissue. It usually comes up with nothing actionable, but if any of his metastases are easy to reach, it might be worth a try.

I’m very sorry for the lack of info needed for a response. I’m just trying to find my way through this and am not always exactly sure what needs to be mentioned. This journey is overwhelming for me at times.My husband was hospitalized with another illness that year and it the process to dealing with that, tests showed a troubling level of anemia. Medication was tried but eventually we were referred to a hematologist/oncologist who diagnosed prostrate cancer by looking at scans, PSA levels and other bloodwork. No biopsy was done. At one point during chemo, a transfusion was needed. Levels continue to be good there. Shots for strengthening bones and reducing testosterone were given during treatment and continue now.

I will pursue the biopsy issue at next week’s appointment. We are now on our 3rd oncologist. The first two were Fellows who left for other opportunities. The one we have now is the one who prescribed Xtandi and is part of the permanent staff.

First priority has to be a high res scan (eg Ga 68PSMA). You need to know where it is to figure out where to go.

I was on Xtandi (enzalutamide capsules) for about 12 months in 2020. At first Xtandi seemed to be working but In late 2020 my PSA also inceased from a low level to 50. My oncologist recommended Lu-177. I am in Australia and had my first infusion of Lu-177 in February 2021 and my PSA reduced from 50 to 3.5. I had my second infusion on 8 February 2020 and I will get the results later this month. Lu-177 is quite expensive but I had no sifde effects with the treatment. Best of luck for your husband with what ever treatment is recommended.

My PSA has also increased since on Xtandi. I have some metastasis and have talked to two oncologists. Both told me I have a year to live. They want to use Docetaxel. What is life like after rounds of Docetaxel?

I would suggest you open both eyes and kick your doctor in the giblets. He's got some giblets predicting you have a year. Show that bastard his prediction in 15 or 20 years.....If your doctor is a female, then kick her husband in the balls.....

It's both..... cause when the doctor announces that you have Pca, he yells out GOALLLLLLLLLLLLLLLLLL!!!!!!!!!! ........

My husband’s psa has increased on Xtandi, but is stable at 25. Dr said it being stable is important. He’s got scans coming up and we’ll know more after that. He’s had genetic testing which showed Lynparza will be effective when it’s time for it. It’s just so hard to wait and wonder. This disease requires a lot of patience.
