Father just finished 6 rounds of docetaxel, 9 Glisson, diagnosed in may 2020. Mets in the bones and spine. The original PSA was 135, now 0.7. Gets Lupron and Zometa. Should he start taking Xtandi now, or wait for the PSA to start rising? Opinions of different oncologists were divided.

They often wait until PSA rises above 2.0 or new/larger metastases appear, but I suspect that it is best to bring out the big guns as early as possible - either Zytiga, Xtandi, or Erleada - whatever his insurance will cover.
Started mine when PSA stalled and rose during chemo. after cycle #4 (tested before # 5 ). Resumed 25% drop/cycle--pushed though 9 cycles which was at least 1 to many. Knocked down cancer but ended up with permanently dead nerves and painful neuropathy in hands and feet.. now on 1/2 dose.
My Doctor had me start the xtandi one or two weeks after my chemo, he wanted to be very aggressive with the cancer. 51 months and counting. Good luck there fellow warrior 🙏🙏🙏
I started Xtandi early on, per Snuffy Myers, part of his aggressive protocol, so far so good PSA remains undetectable six and a half years after dx, G9, Stage 4. I’ve been on Xtandi five and a half years now and was taking it during chemo per CHAARTED.
Extensive bone mets and some abdominal lymph node mets. Started Xtandi 2 weeks after 8 cycles of chemo. Chemo brought psa from 59.9 to about 7.0 About a year to get to <0.1 Xtandi for 38 months now.
