My latest psa is 0.03 after almost 3 years on Xtandi. However my free psa is 0.33.
What does this mean.

I have read some papers claiming a reversal of the %FPSA marker meaning. That is, at diagnosis a higher percentage of the free to total PSA ratio indicates a lesser probability for cancer, hence, higher is better. After primary treatment, with a cut off value of 15% (some state lower values) the meaning is reversed to less is better, claiming that a higher percentage is indicative of a more aggressive PCa. To my understanding, though not explicitly mentioned, the latter apply under no additional treatment after the primary one. In your case, the ADT renders the %FPSA marker incomprehensible, i.e. 0.33/0.03 =1100%! Anything above 100% defeats any logical comprehension IMO.
This may be a lab error or a misinterpretation due to different units used such as nmol/dl vs ng/ml etc. I will repeat the test.
It means you should stop getting free PSA tests. It has no meaning after your original diagnosis.
It means your doing great! Ignore the free psa tests and focus on the .003. Don't stress yourself.
No such thing as a free lunch and free psa test for you..............
