Lupron while on Zytiga.

Lupron works by blocking testosterone manufactured in the testicles. Zytiga blocks testosterone from being manufactured in the testicles, Adrenal glands, and cancer cells themselves. So, while on Zytiga, what's the benefit of continuing Lupron? I'm asking because due to the coronavirus outbreak, I'm considering postponing my next Lupron injection for 8 - 12 weeks. I have been on Lupron continually for five years. Strange thing is; after 5 months of Zytiga being added to my Lupron infusions, my testosterone has gone from 16 ng/dl to <7 ng/dl . Thoughts? Would it hurt to postpone my next Lupron injection for 8-12 weeks.

Lupron is much better at shutting down testicular production, which is the main source (it shuts off the signaling pathway and not just an enzyme). There were a couple of small trials of Zytiga+Prednisone alone, which did well. But it is far from the kind of evidence you would probably want before foregoing Lupron entirely.

What would you consider the risk, if any of missing a 3 month dose, then resuming Lupron which I have been on continually for five years? PSA currently 0.2, testosterone currently <7 ng/dl.

I don't know. After that long on Lupron, it might take the testicles that long to recover. Can't you get the shot from your PCP?

IMO the risk is minimal....as inavertently you are going into the realm of kind of Intermittent Andogen therapy.

Testosterone does not shoot up overnight and neither does PSA. Only fear shoots up that fast.

Removing excessive suppression of androgen receptor..your testosterone will gradually rise and then, the partially androgen resistant cells might turn back into Androgen sensitive cells ...thus prolonging your androgen sensitive status longer. This is what famous Oncologist, Dr Lawrence Klotz 's research indicates.

PSA and T must be monitored every month ..ideally every 2 weeks during OFF period. to watch the behavior of your cancer. If things do not work out and PSA rises fast....you can quickly go back to Lupron again...can anyone stop you from that.

Whats the harm in trying INtermittent Lupron therapy...just vigilance is needed.
