Good Morning Everyone.... I have a couple of questions and I know you can help me out. Hubby’s PC recurred at the end of 2016. Since then he’s had casodex, lupron, 6 of taxotere, Zytiga, 9 of cabazitazel + carboplatin, Xtandi, Keytruda, 6 more of cabazitazel + carboplatin, 5 days of radiation for pain in his hip, and has had the first of Radium 223. He has both adenocarcinoma and neuroendocrine (diagnosed in March of 2018 from a bone biopsy). He hadn’t had any pain until this year. The radiation helped with the hip pain. Radium was on 2/21 and since then he has had much more severe pain which now has seemed to pass, but now he has had debilitating fatigue to the point where he can barely stay awake and do the most minimal of tasks. He also started taking tramadol at the time of the radium but has since stopped. And he has had diarrhea off and on which I attribute to the Radium. Has anyone else had debilitating fatigue from Radium? Any suggestions for how to help him combat the fatigue? It is far worse than Lupron fatigue or chemo fatigue, and it has him severely depressed since he’s unable to go fishing or ride his bike or do much of anything. It doesn’t help that I’m out of town and unable to help pull him out of the dark place he’s in at the moment.

Any and all suggestions are greatly appreciated.

Radiation-induced fatigue is hard to manage. Exercise can help. Anti-nausea medicines may help. Because Ra-223 is excreted through the bowels, something with a protective coating action, like pepto-bismol may help a little. There is a medicine, mesalamine, that is specifically anti-inflammory in the bowels that may be useful. Here's a discussion of it, if you are interested:

Tramadol was horrible for me. Couldn't think ahead or behind. Really a problem.
