My husband has extensive metastatic cancer and is being treated with zytiga and prednisone . He is getting xgeva shots once in awhile as They won’t prolong life but may help prevent breakage. Should I try to be more vigilant about getting these shots monthly. It has more to do with our travel schedule that we have missed them! Opinions???
Xgeva shots: My husband has extensive... - Advanced Prostate...
Xgeva shots
It is dependent on the dosage. Normally Xgeva for men is every 3 months whereas Prolia the same med for women is administered every 6 months.
Denosumab is effective in preventing skeletal events and associated pain , in castration resistant prostate cancer. The trial comparing denosumab to zoledronic acid used 120 mg of Denosumab every 4 weeks.
Did his DEXA scan show osteopenia?
Minor variations to the timing of doses may not make much difference. It builds up to a steady serum state over a 6 month period, and that level can be higher or lower, depending on the size of the individual.
"With multiple subcutaneous doses of 120 mg once every 4 weeks, up to 2.8-fold accumulation in serum
denosumab concentrations was observed and steady state was achieved by 6 months. A mean .... serum steady-state trough concentration .... was achieved by 6 months. The mean elimination half-life was 28 days."
"Body Weight: A population pharmacokinetic analysis was performed to evaluate the effects of
demographic characteristics. Denosumab clearance and volume of distribution were proportional to body weight. The steady-state exposure following repeat subcutaneous administration of 120 mg every 4 weeks to 45 kg and 120 kg subjects were, respectively, 48% higher and 46% lower than exposure of the typical 66 kg subject."

I get an Xgeva shot every month and was told I need to take 1200 mg of a calcium supplement every day to strengthen my bones to make the Pca difficult to metastasize to the bones.
My husband has extensive Mets to bones and gets an xgeva injection once a month. I’m impressed you’re able to travel extensively.
It is possible that Xgeva + Celebrex may extend survival, FYI (link originally posted by Tall_Allen IIRC)

I am on Xgeva and recently started Celebrex. At the very least, it seems to be helping with the leftover pain after chemo.
My SO just had his MO apt yesterday. He specifically asked why he is not given Xgeva every month. MO said he has several patients with the Jaw problem (can't remember the exact name) from Xgeva so he rather keep Xgeva every 3 months.
When he asked how often he should be getting Bone scans, MO said once a year would be enough.
He does take vitamin D3 and calcium daily.
Stage 4 here August of 16, been on the Xgeva shots monthly since then. Have severe Mets in my ribs and spine. Doctor has been happy so far. Will have another round of scans in June. To make sure we’re doing ok. Ribs still hurt but haven’t managed to break any thing as I am very active and always lifting more than I am supposed to. Fight that monster 🙏🙏🙏
I was getting xgeva injections monthly until my bone density scan came out normal so I switched to quarterly.

For me monthly was not an option because of the 10 days of discomfort following each injection. I have now been getting it (xgeva) every 90 days since Oct. 2018. (Bimonthly prior to that)10 painful days outa 90 not so bad. My doctor attempted a bone marrow biopsy on me back in February which did not go well as they could not
Get the special needle to pierce the bone in the hip. Said I had the hardest bones he had ever tried to biopsy.
I was on Lupron (Eligard - generic) for 6 1/2 years and after 3 years I had a bone scan. I needed Xgeva shots every 6 months for 1 1/2 years. hen my bones were OK and I no longer have Prostate Cancer. It was never told to me that I needed it every month, and skipping it could happen with permission from your doctor.
Elgie was getting Xgeva monthly for 1st couple years then he went to every 3 months for couple years. Doc said bones had a miraculous healing. Now he gets them every 6 months.
