Good morning all

My husbands PSA is on the rise. In 2017 he was diagnosed. Gleason 7/8 Did 42 radiation treatments and Lupron injections. 1 year later PSA on the rise. Had Axumin scan that showed Pca spread to pelvis and retroperotonial. (One of the unlucky 5 percenters who’s prostate cancer is outside the prostate). We decided on radiation treatments to both area. Started Xtandi. 4 weeks later he ended up with lymphadema left thigh.

10 months later the Xtandi is no longer working. First of all his PSA never went below 0.7. Once it reached that number it went to 1.15 then 2.14 and recently 2.95. Had CT scan of chest, abdomen, pelvis and bone scan. Both confirm spot on left hip. Ct also showed spot at L4 vertebrae. Dr wants MRI of both areas to be sure L4 is what we think it is. He is recommending radiation again to both areas and change of meds to Erleada How do we pick next treatment plan? At Dr appt Dr Oh said, “I’m your dr and I can choose for you....let’s do Erleada.” Literally putting your life in someone else’s hands So scary but that is his recommendation

Lastly, we had Foundation do gene mapping. Dr talked about ERG Fusion. Would like input on my husbands next plan of care from anyone who may have heard of or tried Erleada. As well, some help with gaining a better understanding of what ERG Fusion means. Dr Oh said there is no formal treatment Re:ERG Fusion.

He is right that there is nothing to be done about ERG fusion - it's a common part of the genetic breakdown that occurs in tumors. i'm impressed that he is able to get Erleada for him - probably from a clinical trial. Dr Oh is one of the best.

Ouch. This does not sound good. So nothing will work on my husbands cancer cell tumors to slow the progression?

Oh my goodness ‘no’. I’m hanging on to every word!

LU-177 is of interest to me as well as ACT-225 (I think) and Radium 224. I’ve never done so much investigating in my life But I believe I have to study hard, read a lot and listen to you and a few others on this site who have a WEALTH of information that we all benefit from. My heartfelt thanks for your care , concern, time and efforts.

I am taking Erleada for about 8 months. I believe Erleada is only approved for NMCRPC. I don't know how your Dr is getting approval for Erleada with your husband having mets.

Can you please tell me what this means? NMCRPC.

Someone asked why my husband would receive Erleada when he has the above and how is he getting that Med?? Need some guidance and understanding

I believe that would be Non Metastatic Castrate Resistant Prostate Cancer.

Was the Ca outside the prostate at the time of Dx?? I'm not sure what you mean when you refer to the unlucky 5%

Upon initial diagnosis we were told that 95 % of prostate cancer is curable unless it is outside the prostate. When his PSA began rising one year later and he was diagnosed with prostate cancer outside of the prostate that he is in the 5% group.

Thanks Nal. I misinterpreted IV and thought it was a reference to the Roman numeral IV - how dumb is that? I should have known it was meant to be IV as in healthcare. I spent a lot of years in healthcare but I have never encountered IV administration of alpha-lipoic.
