Zytiga failure after just 6 months and subsequent RP. Continuing on Eligard. Just started 50 mg Casodex daily to control accelerating PSA, 0.15 11/22/18 to 0.55 2/12/19. Is this the right dose, as I have seen various postings of up to 150 mg?
I am pending application for Provenge. Also Olaparib outside of clinical trials, as I am BRCA2+. Hitting it hard.

If Zytiga fails I am afraid Casodex will not help. You could try to get into a Darolutamide trial, this apparently has no cross-resistance with Zytiga.
The 150mg Casodex are used for Casodex monotherapy. If used in combination with Eligard 50 mg should be enough.
Yes, Olaparib could help in your case.

50 mg is the dose when given with Eligard, 150 mg when given as the sole therapy. Xtandi is a much more powerful anti-androgen that might work for a short while.
Did they try swapping out the prednisone for dexamethasone while on Zytiga?? Some have had success adding indomethacin with zytiga to continue the med--squeeze greater efficacy out of the drug--prolong the time...just some thoughts----see below:
Appreciate your reply and input. Yes, dexamethasone replaced prednisone for 3+ weeks while continuing on Zytiga, but PSA unfortunately has continued to rise. In addition to soon going on to Provenge and Olaparib, waiting for insurance approval for auximin PET scan to find any possible additional mets. Also have genetic report from Foundation Medicine under review by my medical team at CINJ, including their Precision Medicine dept, for treatment options.
Could add the indomethacin if your stomach and kidneys can take it...sorry to see you in this spot...Good luck, brother... may your team come up with a new plan that works great for you....sound strategy---precision medicine--targeted therapy....
One last thought....depending on number of mets--there are stereotactic radiation trials for oligometastatic disease--page 4 and SABR-COMET-10 is unveiled to treat from 4-10 lesions...one other way to attack the beast...
