Hi, my name is Mike and I am new to this site. After having HIFU two years ago, my psa stayed low for a year then started to climb. The last PSA showed a tripling in 6 weeks (from 1.16 to 3.48) so I knew something was wrong. After a clear 3T MRI, I had an Axumin Pet scan and got the results last week. My cancer has spread to 3 local nodes. Trying now to decide on a course(s) of action. Am juicing, eating plant based diet, and doing several other supplements/herbs. If anyone is in a similar situation I'd love to know how you proceeded. Urologist wants me to start on ADT to slow it down.

You can have salvage radiation with long-term ADT to only the pelvic lymph nodes. Hopefully, that will be curative.

MikeNorris - I would second the ADT and get PSA as low as possible, .50 or lower if possible, before any further treatment is administered if going for curative. The weaker the cancer the better the treatment will work. If your at an average hospital with general oncs and standard of care practice you need to seek a center of excellence with a doctor that excels in the prostate cancer field. Aggressive therapy will most likely depend on your age and current physical health. If mets are limited you still have a shot at curative but you have to stack the odds in your favor to the best of your ability.
I highly recommend The Urology Department at The Mayo Clinic in MN. There are other great centers but speaking from my own experience they treated me and I wouldn't change a thing. In 2016 I was diagnosed with Gleason 9, bulky disease in pelvic and abdominal nodes and PSA at 286. I now have report of no evidence of disease and pray every day it stays that way. Hi Mike, I’m in a fairly similar situation. Original Gleason 8, treated with ADT and EBRT, then had a relapse and went for a whole gland HIFU. Fast forward to now, while my prostate bed is clean, pet scan has discovered a bone met in the Lumbar. ADT may buy me a lot of time but I really want to get rid of the devil. Am now considering radiation. Went to the PCRI conference in Sept and talk there was all aggressive treatment, particularly against oligometastasis. In your case, with 3 nodes you should belong to the group. You may want to research more into the subject, and at least should watch a YouTube video of Dr Eugene Keon of Mayo Clinic talking about oligometastasis.
