Pirads 5 lesion treatment?

Hi all, I have just been diagnosed with prostate cancer but my ride has been crazy. I had a biopsy about 6 years ago because of elevated psa’s. That turned out to be negative so we just followed the trends. Last year psa came back 4.04 and my urologist orders a mri with contrast. The radiologist see two lesions and rates them pirads 2 and 3. I them get a prostate fusion biopsy that has an oncotype number of 23. So I consult with two other urologist and the agree on active survallance but the want there radiologist to review the mri. Guess what? They read it as a pirads 5 lesion and prostate size of 54. I am suppose to get another fusion biopsy but am not holding my breath. I am gearing up for a prostatectomy but am really scared of the side effects. Has anyone had the surgery and what has been your results?

What? you don't have a prostatectomy based on a PIRADS score. That's just crazy. PIRADS only identifies SUSPICION for cancer. But you have to have a biopsy to confirm it. Make sure they take 4 or 5 cores from the suspicious area.

I agree 100%. And make sure to get an MRI guided biopsy. My random biopsy was 0-14. My mri guided biopsy was 3-3 Gleason 8

Raises Hand.. I've had the old fashioned RPD (cut me open) at the same time I had two hernias repaired. In Friday morning out Monday morning. Little pain but wearing a foley for a week is a nuisance. You'll need depends for awhile cause you'll probably be leaking. Do your kegels now, the more the better. As far as little willy is concerned say goodbye to him and just call him "w" from then on. But before you have the RPD make sure you want it and need it. Get yourself an independent (not affiliated with your Urologist) Oncologist who specializes in Pca, the sooner the better. (By the way tell us where your located).

Good Luck and Good Health.
