My son is 7 and he hasn't been able to take guanfacine, clonodine (hallucinations), stimulants (too much reactivity and anxiety) and now we are on to qelbree.

I’m not sure about quelbree, but did you know you can take a genesight test for him? It’s just a swab in the mouth and will tell you how adverse the side effects may be with his body in particular. I just had one done, my partner loves the med it recommended and my daughter is in the process of getting one.
I can’t speak on qelbree but my son’s explosive behavior has been helped by Prozac. He just turned 8 and has been on it since early July. He started at 10mg and after about 6 months, he’s up to 20mg a day. And I agree, a genesight test is a huge help. I found out my son has a methylation problem because of it and now give him supplements to replace his folate and b12. Guanfacine didn’t work for us either. I also give him Magnesium and L-theanine an hour before bed to calm him down and help quiet his mind. Good luck.
Did you try immediate or extended release guanfacine? Just curious.
Also, our doctor had us do the genesight test, it was kind of nice to know and was slightly useful in thinking about our options, you may just want to make sure your insurance covers it beforehand since I think it can be expensive otherwise.
I can only tell you our experience with Qelbree as every body will react uniquely. My daughter is on Qelbree right now, for the second time. The first time side effects were weight loss due to loss of appetite. We tried some other things and came back to Qelbree at a lower dose and it’s been good so far. It works well with helping her think more clearly and focused. So far no loss of appetite. She can be a little more aggressive on meds- she’s 18 and when I say aggressive I mean a little sassy and spirited. Not mean or physically aggressive. So far so good.
I highly recommend looking in to Dr. Amen’s 7 type of ADHD, and the different types of treatments depending on the type of ADHD brain. I know his work is disputed but it makes sense that he looks at the brains he treats so they are receiving the correct treatment. You can take a quiz on his website noting the symptoms that will help with determining best line of treatment without a scan. What works for one person may cause another to be suicidal, angry, more impulsive, ect because we are guessing when treating our children. I will also note he is not anti-medication.
Curious if you started Qelbree. We just finished 1 month and he seems to be able to focus more and his grades have improved. Not sure much else has changed in regards to impulsivity and anger.
