A randomised phase II trial of 177Lu-PSMA-617 (Lu-PSMA) theranostic versus cabazitaxel in metastatic castration resistant prostate cancer (mCRPC) progressing after docetaxel: Initial results are much more positive than earlier reported in the USA...now this is a Phase II trial (Phase III trials are scheduled to start soon) and its not 'blinded.' It also has 'patient selection bias,' meaning patients were chosen who would be more positively treated...BUT what purpose does it serve to select patients for a treatment that wont work on them...never have understood 'patient bias,' but I am not a researcher...
Bottom line is that, IF you fit the treatment parameters, Lu177 really works...it works now in the most desperate of PCa patients (castration resistant metastatic PCa)...so lets keep our eyes open to this treatment...my take is, if PSMA PET is so radically better because Ga68 rides on a protein that selectively 'binds' only to the antigen on PCa, then why would Lu177 piggybacking on the same protein not work just as well...only it ahnialates in a millimeter width blast pattern in lieu of emitting a photon to show where the PCa is located...dont get it...AND is not a focused radiation treatment that targets only the area of PCa growth not 'healthier' than a general Chemo which floats up against all your body cells?
From my viewpoint once this treatment is shown in Phase III that it will prolong the life of men with the worst metastatic, castrate resistant PCa, its use will be widened to include early treatment of PCa...i.e. as a prophylactic...again, I am not a Doctor, but if you treat anything, even car trouble, before its too late the efforts are much more effective, less expensive and it prevents complete failure...but we are not in charge here...keep an eye on Lu177...Cheers
