Can you resume pred at the last dose 5mg
Or is a 40mg higher dose the norm???

A 40mg higher dose would be to hit any inflammation really hard and be sure it is enough. There is no need to titrate a pred dose up like you do with some drugs - so take what your doctor told you to. 5mg is a very low dose and won't deal effectively with inflammation as a start.
