Some of the countries with endemic iodine and/or selenium issues seem to take thyroid very seriously. I'd include Japan, Croatia, China and Germany (though that is just a selection that I have read about - other countries may be just as serious).

Maybe ironically, the British Geological Survey was involved in some of the important work in China.

In Japan there is at least one dedicated thyroid hospital.

What seems to me to be a major issue, one which goes some way to explaining the dissatisfaction common in the UK, is that autoimmune thyroid disease appears more prevalent among northern Europeans. And AITD has consequences and complexities which mean that the direct treatment of deficiencies and/or excesses does not by a long way resolve the issues caused.

In terms of academic research I have been impressed by the volume of papers coming out of Italy and China. Russia has a done quite lot of research work but much is inaccessible to us, not least because a lot is not on-line and it is virtually all in Russian. In addition to countries already mentioned, I have seen papers from South Korea, Iran, Israel, Saudi Arabia, France, Canada, Sweden and Jordan - and probably many others.

But when it comes to actual treatment of real patients, well, I think I would have to choose Japan. Although the USA does appear at its best to be quite good, at its worst it sounds pretty dreadful. And you can see some evidence of that in the mountains of posts from there which dominate the on-line thyroid world.
