Hi again I had posted earlier in the week and now have my full advanced thyroid results below could someone interpret these for me. I'm on 225mcg of levothyroxine so looks like I'm being over medicated but I'm positive for thyroid antibodies which I've never been aware of.

FT4 - 32.1 pmol/L

Thyroglulobulin Antibodies TgAB - 20.7 KIU/L

Thyroid Peroxidase TPOAb - 181.0 KUI/L

TSH - 0.01

Thyroxine T4 165.0

Triodothyronine FT3 - 8.1

Please add reference range for each test - numbers in brackets after your result.

Did you also tets key vitamins - ferritin folate, B12 & D3?

What time of day was this test and did you leave 24hrs from last dose of Levo?

By most ranges you would be over replaced. Suggest you reduce by 25mcgs and restest in 6-8 weeks.

Positive antibodies mean you have autoimmune hypo or Hashimoto's. Worth trialling strictly gluten free diet.

Start a new post when you have the vitamin results.

Doesn't matter what time of day the blood draw was. With those high Frees the TSH is obviously going too be suppressed at any time of day.

Yup, looks like a Hashi's 'hyper' swing. Your dose would have to be extremely high to get those results, which is highly unlikely.

So, the high levels are only temporary. The excess hormone will eventually be used up/excreted and you will be hypo again. But impossible to tell how long that will take.

Whatever you do, don't let them put you on antithyroid drugs - carbimazole etc. You are not truly hyper, nor are you over-medicated. And on anti-thyroid drugs you will become very hypo very quickly.

But how do you feel with these levels? If you feel hyper-ish the best thing to do would be to stop your levo for a while until you feel hypo again. But do it on your own. Don't let your doctor see these results because if a doctor reduces/stops levo, he doesn't like having to reinstate your dose again. And none of them seem to have a clue about Hashi's 'hyper' swings!

Thanks so much for your reply I'm on 225mcg of levothyroxine have been for a few years now and thing is I don't feel hyper ish It's the opposite I feel like my tank is empty and I'm running on fumes, hair loss, weight gain, joint pain low mood all the usual symptoms but worse than usual i recently have a pressure feeling around my thyroid and feel like I'm struggling to swallow at times don't know what's going on just feel fobbed off with my gp

OK, so that is a high dose! What about previous blood tests? Have your levels always been that high? I wouldn't jump to any conclusions until you've done a bit more investigation.

My last few blood results tsh and t4 have been stable as gp says so no need to reduce or increase dose. But this is first I have been tested for t3 and the antibodies as I paid private for them. Gp wants to reduce my dose because my bloods are showing signs of hyper

Yes, I expect he does! But he should know that if you're hypo you cannot suddenly become hyper. Taking thyroid hormone replacement doesn't do that.

You could be over-medicated, of course, on a high dose like that. But without the numbers from previous tests it's impossible to know.

What on earth does he mean by 'stable'? Being 'stable' has nothing to do with anything. You could be stable at the bottom of the range just as you can be stable at the top, meaning that your results are more or less the same with every test. But that has nothing to do with being optimal. And that's what we want. And we need to see the numbers - results and ranges - to know what's going on.

I actually changed surgeries as i wasnt happy with care from my last gp .I have an appointment on Monday so I will ask for a print out of my previous results. Can I ask my levels of antibodies TPOAb of 181 does that indicate hashimoto's I've never been told that I have hashimoto's

I would imagine it does, yes. The usual cut of point is about 38 so that level is bound to be positive.

UK doctors don't tend to call it Hashi's. They call it Autoimmune Thyroid Disease, or Thyroiditis.
