Prescribed T3 at long last, advice on when to take and how to take please

My private endo has confirmed I am a poor converter and has thankfully prescribed Thybon Henning which should be arriving any day now. I currently take my vitamins with breakfast - B-complex, Vitamin D, VitaminK2 MK7 and take my levo at night with a magnesium spray. My prescription states 5mcg to be taken twice a day. Reading earlier related posts, I note that SlowDragon suggests taking just one dose in the morning for a week or so and then increasing gradually to the full amount, which advice I will follow. My question is this, will I be okay taking the T3 in the morning with breakfast? My other question is, when I start adding the second dose, what would be the best time to do this? My evening meal is usually around 5pm and I eat very little else before bed, maybe a biscuit or a piece of fruit. Thank you as always for your words of wisdom !

My prescription states 5mcg to be taken twice a day.

That was my instruction when starting and I would never do that again!

Yes it’s true I was very under-replaced at the time, so lots at play.

But for me, it was way too much, all at once. It actually took me 9 months for constant palpitations to subside, and 6 months after that I’m still figuring out if I need to ultimately take less than 10 a day anyway.

Right now I’m trying out 7.5 a day and although it’s never a straight line, it’s far better than how I felt on 10.

Just my experience.

Oh gosh, that's grim. I hope you get the best dosage for you sorted soon. My thinking is to ignore the 5mcg twice a day and your experience certainly endorses that. Thank you for sharing.

Looking at very helpful profile

I reduced my levo from 112/125 mcg to 100/125 mcg and have just retested with MMH

January 2025 results:

TSH 0.42 (0.27-4.2)

FT4 18.6 (12-22) 66%

FT3 4 (3.1-6.8) 24.3%

So hopefully Endo hasn’t suggested reducing Levo?

So Thybon Henning is 20mcg tablet

You will need to cut into 1/4’s to get 5mcg

I would initially start with 2.5mcg (almost a speck of dust) …taking first dose waking ……wait 30-40mins before having anything other than water

2nd 2.5mcg mid afternoon ish - say 4pm

Assuming that’s ok

After 3-4 days increase morning dose to 5mcg

And waiting a few more days before increasing afternoon dose to 5mcg

In few weeks

You can then experiment with taking last dose at bedtime with Levo

Retest after 6-8 weeks with last 5mcg dose T3 at 8-12 hours before test

Aah thank you that's helpful SlowDragon. So 2.5 mcg will be minute, a speck of dust as you say! I would have started with the 5mcg so thank you for clarifying.

Regrettably the endo has stated I should reduce my levo from 100/125 to 75/100. Which is annoying because the GP prescribes 125mcg daily for me. I'm concerned though if I don't follow the instructions from the endo, it will give a misleading result when I retest in 6-8 weeks. My feeling is at the moment to follow what he says, do the test then tweak the levo if I need to. That might of course entail going to the GP and pleading for a reinstatement of a higher levo dose ........

Well as you are seeing Endo privately……GP doesn’t need to know about reducing dose

Just continue collecting current prescription

But I guess you might have to reduce levothyroxine as endo requires ….personally I think you have done that already with recent reduction

You can cut Vencamil quite easily

Do you get 25mcg Vencamil too?

Initially only reduce Levo by 12.5mcg for 3-4 days BEFORE starting on 2 x 2.5mcg T3

Then reduce levothyroxine by further 12.5mcg before increasing T3 to 5mcg x 2

You’re are likely to have to increase levothyroxine back up after next test

Yes I get 25mcg of Vencamil so that will make the levo reduction before starting the T3 a little more straight forward. The endo has written to the GP informing them of the proposed reduction. However, I'm hoping I can squeeze my usual prescription in before the GP gets around to changing my prescription. A levo buffer is always welcome to us hypos ! Thank you for your clear guidelines.

Just to quickly add a small thing. If you find it very difficult to split the tablets into 8. Roseway Labs are a compounding pharmacy and reasonably priced. I have 2.5mcg made up by them and it also means you can choose lactose free etc if needed. Best wishes to you.

You can cut Thybon to 2.5mcg with a semi-decent pill cutter, I did for quite a while. It's not always easy/perfect, but just about doable.
