I’ve not heard of someone on 4 grains before, that sounds like a very high dose, I’ve also heard that Thyroid S varies quite a bit. Could it be a weaker batch and could your absorption be low? What dose of T4/T3 did you take last year?
But by that calculation you’d need 240mcg T4 and 36mcg T3 which doesn’t sound right to me.
Lower down the page it says some NDT equates to 100mcg T4 but 400mcg T4 is a huge dose. Again I’d be very concerned about suggesting someone take that much even short term.

What doses of levothyroxine and T3 were you taking before going over to Thyroid S?
I think I'd be cautious. Being a little under for a week is possibly a lesser evil than taking too much and going over the top.
The actual hormone content of a desiccated thyroid product never seems a particularly good guide.
Nor does the one grain to 100 micrograms of levothyroxine. That seems to be a not unreasonable idea when converting from levothyroxine to desiccated thyroid as it would always tend to underdose. But going the other way, it could seriously overdose.
Previous discussions have ended up with different views, but somewhere around one grain of desiccated thyroid being somewhere near 60 to 75 micrograms of levothyroxine seems the most common. However, I do not know how best to incorporate T3 into any equivalence. And the truth is, there is no magic formula, no exact equivalence.
I take 3.25 grains of Thyroid-S daily and one grain is supposed to be 38mcg T4 and 9mcg T3, so in theory the equivalent should be 152mcg levothyroxine and 36mcg T3, but I doubt it's as simple as that. Also I don't think you're supposed to switch straight over, you should probably start with a lower dose and build up, which in a week isn't possible.
Do you still have any Thyroid-S left? If so it would probably be best to spread it out over the days until the new order arrives. If you don't have any left you could take nothing and see how you are, and if necessary take a small amount of levothyroxine, but I'm really not sure what is advisable. If it were me I'd probably take nothing and hope for the best. How sure are you though that your order will arrive next week?
I take Thyroid S and my order was despatched on October 12th but I still haven’t received it. I got in touch with my supplier and he said the post had been delayed for some reason, so I will have to go onto levothyroxine in a few days if my NDT doesn’t turn up. In the pandemic when I couldn’t get Thyroid S I got levothyroxine from the doctor. I take 2.75 grains of NDT and I took 100 mcg of levothyroxine at that time.
