Advice please for next Roseway appt

What size T4 capsules should I ask for next? I currently take 125mcg split as 2 x 50 and 1 x 25. I'm taking Roseway's compounded T4 capsules so they can't easily be split. I had to test this week as I need my appointment next week in order not to run out, but I only completed my transition from NDT on the 6/4, so just over a week before the test. My plan is to keep on the same dose til at least 6/5 then I assume I will need an increase as the results look pretty hypo to me (and I have hypo symptoms). Test was done as per guidelines (note in case it's useful: I have sometimes been a "reluctant bleeder", drinking loads of water the day before and that morning meant I only needed one lancet and actually had trouble getting the plaster on. What a weird thing to be happy about!!)

So to save some cost (although I'm told Levo is cheap, there seems to be a minimum charge for compounding - makes sense but these initial stages are expensive!) I'm trying to figure out what size capsules to go for, assuming I will need some more 100s and to go up to 150(?) in a few weeks.

Just trying to plan in advance - thanks for any advice as always.

without your results its impossible any make suggestions. I buy my compounded T4 from Roseways and find the prescriber very helpful in arriving at a joint decision on an increase in dose or changes to medication depending upon my symptoms.

hello, you look still very hypo to me on those results and symptoms and your FT4 is low in range despite the dose.

Hmmm… are you absorbing properly?

Not eating for an hour after taking, taking with a glass of water? No magnesium within 2 hours etc. I’ve made that mistake myself before.

They may suggest T4 T3 combo, but I’d want my FT4 higher in range before starting T3.

They also like to wait 12 weeks, so it could just be timing.

Hi Regenallotment lovely to hear from you. I think I've been doing everything right: levo first thing with a glass of water, breakfast 2.5 to 3.5 hours later in general, supplements with breakfast (or after dinner, for magnesium and D3/K2).

Yes, I was also thinking to try to get the fT4 higher in range before thinking about whether T3 is needed. Just not sure how to go about increasing the T4, I'm guessing 25mcg at a time and to wait 4 - 6 weeks then retest. So I probably need to stick with the 50's for the time being, it's more flexible. I think... Or 100s and 50s? arTistapple I am so seeing your point about making the plan before you make the change. My poor brain is a bit scrambled! If I've got this wrong and can increase already I'd be very pleased, but I think I have to take it slow, I've learned that lesson.

Ah yes so 100s and 50s next I guess.

I became a dab hand at splitting capsules. Prised apart the two halves, emptied roughly half into a glass of water and reassembled, chugged the water and had the other half later. You get surprisingly good at it with daily practice. 🌱

Another idea, I found their compounded T3 stronger than the Thybon I’m now taking (also from them) I wonder if their T4 is somehow weaker or less well absorbed with whichever filler it is? Perhaps ask if others have experienced any difference?

Oh and…. Sorry if you have already covered this but vitamins and minerals… where are you on those? I don’t absorb or convert very well if my ferritin is low. Did you test those too?

Thanks for the thought :) all tested in Jan and levels looked ok except for vitamin D, I could probably do with increasing my dose of that but haven't yet. CRP tends to be high ish though and I've heard that can cause a false increase in ferritin but I'm not sure what that actually means!

May I ask why you transitioned from NDT to levo?

No suggestions I’m afraid.

No worries, thanks for the reply anyway :) There was some discussion about it in my last post; basically I started things a bit backwards and have never tried Levo-only so don't know how good my conversion is, also ever since starting NDT I've had an increase in joint pain & muscle stiffness; my cortisol levels tend to be low and felt worse lately on NDT (hard to wake up in the morning). I've now learnt that NDT doesn't suit all Hashi's patients... So it seemed worth trying levo only, at least, then some additional T3 if needed.
