Today I had a hand addressed envelope in the post. It contained a bag for the blood test and apparently in small print at the top of the bag, it is from my Endo. I am guessing this is the form I needed for the blood test I had in November,

There was nothing else in the envelope.

So next time I feel the need for a blood test, I can just book it and hand over the bag as it says to test T3. Bonus!
