1st ever vitamin tests - newly hypo

I’ve followed your excellent advice and got my vitamins tested and was wondering if I could please have some advise on these results.

Starting Levo dose 75 (48kg)

As a reminder I was diagnosed hypo 1 month ago. Since starting Levo have been muscle and joint pain with acid reflux. Scalloped tongue worse also. The reflux I am trying to fix through my diet - the acid watcher book.

I used monitor my health

Todays Results

HB 144 g/l (bottom range 126 - no top)

Ferretin 142 ug/L (10 - 410)

TSAT 41% (15 - 50)

Active B12 146 pmol/L (38 - no top)

Folate >20 ug/L (2 - no top)

Vitamin D 74 nmol/L (60 - 100) replete range

Reminder of Throid Results - tested 1 month ago

TSH 28.5 range (0.47 - 4.2)

Free Thyroxine 11.2 (12-22)

Thyroglobin Antibody 399 (0-115)

Thyroid Peridoxase Antibodies >600 (0-34)

I am due to be retested in 2 weeks.

Thank you very much any advice would be much appreciated. It’s been a bit of a rollercoaster!

Hey there, your results look pretty good... I'm a little bit confused as MMH I thought they usually give a range?... though I've only used them for thyroid bloods. They showed coloured areas on the graph to represent range or if you shrink the page down it changes format and loses the graph

Usually we suggest Vit D above 100?

With no fT3 result from your last test and a low fT4 it's hard to see how this will pan out.... worth insisting they do a full panel if you can twist their arm in 2 weeks time, if not worth doing a private one

If you started on 75mcg and are 6 weeks in you have probably had a pretty horrible ride but hopefully it should start to smooth out a little now, it often gets worse before it gets better and then dips again until you find your optimal 🙃

Eeyore thank you so much for responding.

Yes initially on my iPhone there were no ranges but I found them when I looked on the iPad and added them to my post. Some don’t have a top range it’s left open ended and some do.

I will look to increase my vitamin D 🙂 thank you.

My next set of bloods will be T3 as it’s the first set that my endo will be doing.

Yes it really has been a horrible ride. I felt catapulted from healthy and very active to what feels arthritic and run down. When I had my first appointment with him I was tip top except some neck pain. One week later it all started to go downhill so I hope he can explain what is happening.

I can’t thank you enough, being able to read bios here and share and look up advice makes me feel far less alone on this journey.

🤗 I'm sure if you have been reading us wittering on you'll know it's D3 + K2 MK7 is the better form (helps absorb calcium and direct it to the bones) also about the acid reflux Hypo's generally have low stomach acid and taking something to help with this like apple cider vinegar or betaine HCL with pepsin can be really helpful.... ah also for some taking levo makes them dairy intolerant due to it switching off the lactase response so could be worth trying a few days dairy free to find that one out... it took me years to realize 😕

Thank you Eeyore, I did not know about the D3 plus K2 MK7! I will definitely get that version.

I did know about low stomach acid (learned on here of course 🙂) and I am so confused because ENT put me on PPI which I won’t take especially as I was worried it might make low stomach acid worse especially as I don’t know if I have high or low acid.

Dairy free will be the next one to try if I’m not feeling better.

All the medics seem to think everyone has excess acid and if you don't respond to PPI's they just up the dose! 😵‍💫

Bicarbonate of soda test - how to take the test

Dissolve a quarter teaspoon of bicarbonate of soda in a little water and drink this on an empty stomach. If you have adequate levels of stomach acid, the bicarbonate of soda is neutralised and converted into gas – this means you should experience belching within 3 – 5 minutes after drinking the solution. If no belching occurs, this suggests there may be insufficient acid present.

K2 is more important when taking vitamin D doses of 4000 iu or more, 100 mcg of K2 should be enough.

PAS forum members swear by raw apple cyder vinegar before meals. Lots of info about that from Dr Michael Mosley, The Clever Guts Diet books. PPIs will only deplete your vitamin levels, especially B12. Best avoided. There are tests for low stomach acid, apart from the bicarb one mentioned.

I saw one of the admins posted a vit D calculator which was really helpful so I have ordered 4000 iu with 100 k2 which is a 2 tablet dose so I will take one tablet each day.

You have some work to do to improve things but making a good start...well done.

75mcg is a biggish starter dose so it may have taken your body by surprise and will need time to adjust to the replacement hormone

As you increase your levo be prepared to have a less than smooth journey....this is normal. Things will improve and settle once you achieve your therapeutic dose.

There is no quick fix so be patient ( that wasn't my strong point!)

A month ago...

Your antibodies were high signalling thyroid autoimmune disease/ Hashimoto's. It's a common cause of hypothyroidism Many find a gluten free diet beneficial.

You may find your thyroid hormone levels fluctuate during Hashi swings....this is transient and should settle back down

FT4/ thyroxine is miserably low...you will need dose increases

It's important that FT3 is included in testing

FT3 followed by FT4 are the most important results..

Results for both Frees should roughly approach 75% through their respective reference ranges.

For good health almost every cell in the body needs to be flooded with T3 ( the active thyroid hormone) by way of an adequate and constant supply.

Medics knowledge of the importance of T3 is poor, frequently resulting in poor outcomes for patients.

They tend to focus on TSH which after diagnosis and initiation of treatment is not a reliable marker....though your TSH is far too high

Nutrients must be optimised to support thyroid function/ conversion.

Lower end of range isn't enough.

Vit D: 100-150nmol/L

Total B12....top of range

Or...Active vit B12.....100+

Folate: at least half way through range

Optimal ferritin level for thyroid function is 90-110ug/L

Test again after 6/8 weeks on a steady dose, the results will point to the way forward....minimum TSH, FT4 and FT3

DippyDame I can’t thank you enough for your reply.

Yes I do feel like this is just the beginning of a bit of a bumpy ride but it really does settle my nerves when those of you who have lived this say that it is to be expected. I will hold tight for the dose increase in a few weeks time.

Thanks for suggesting GF am suffering with inflammation and read here about gluten free so started that 1 week ago which is fine for me and I understand that can take a few weeks to start seeing the benefits.

My next bloods are being done by my endo who has requested T3 and a long list of other tests some I know some I don’t. I am looking forward to seeing my results as they are the first ones since starting Levo.
