I had a “flare up” over a month ago! Advanced a thyroid checks done by medichecks! My TPO has gone up from 9 to 11 from being diagnosed 2021! I stopped taking B complex 1 week before test, no extra folate or B12 tabs 🙈 no levothyroxine for 24hrs before I took test and looks like I have an raised CRP which dr on medichecks puts down to infection/inflammation…

Exactly what B12 and vitamin B complex do you normally take

Your B12 is extremely low

You only need to stop vitamin B complex 5-7 days before test

Continue daily B12

In week you stop B complex you need to take a daily folate supplement instead

Vitamin D also too low

How much vitamin D are you taking

Doris11 in reply to SlowDragon

Liposomal (Vitablossom) B complex and 25mcg vit d. Yes should’ve got separate folate and B 12 but forgot and brain all over the place 🙈

SlowDragonAdministrator in reply to Doris11

25mcg vitamin D not enough

Look at taking at least double that

Add separate B12

Nature provides B12 drops highly effective

Doris11 in reply to SlowDragon

As well as my B complex tabs?

SlowDragon profile image
SlowDragonAdministrator in reply to Doris11

Yes both ….your active B12 is extremely low

Low B12 symptoms

b12deficiency.info/signs-an...

methyl-life.com/blogs/defic...

With serum B12 result below 500, (Or active B12 below 70) recommended to be taking a separate B12 supplement and add a separate vitamin B Complex after a week

Then once your serum B12 is over 500 (or Active B12 level has reached 70), you may be able to reduce then stop the B12 and just carry on with the B Complex.

If Vegetarian or vegan likely to need ongoing separate B12 few times a week

Highly effective B12 drops

Or

B12 sublingual lozenges

B12 range in U.K. is too wide

Interesting that in this research B12 below 400 is considered inadequate

healthunlocked.com/thyroidu...

Vitamin B complex

supplementing a good quality daily vitamin B complex, one with folate in (not folic acid)

This can help keep all B vitamins in balance and will help improve B12 levels too

Difference between folate and folic acid

healthline.com/nutrition/fo...

Many Hashimoto’s patients have MTHFR gene variation and can have trouble processing folic acid.

thyroidpharmacist.com/artic...

B vitamins best taken after breakfast

Thorne Basic B recommended vitamin B complex that contains folate, but they are large capsules. (You can tip powder out if can’t swallow capsule)

Thorne currently difficult to find at reasonable price, should be around £20-£25. iherb.com often have in stock. Or try ebay

Other options

healthunlocked.com/thyroidu....

Igennus B complex popular option too. Nice small tablets. Most people only find they need one per day. But a few people find it’s not high enough dose

IMPORTANT......If you are taking vitamin B complex, or any supplements containing biotin, remember to stop these 7 days before ALL BLOOD TESTS , as biotin can falsely affect test results

endo.confex.com/endo/2016en...

endocrinenews.endocrine.org...

In week before blood test, when you stop vitamin B complex, you might want to consider taking a separate folate supplement (eg Jarrow methyl folate 400mcg) and continue separate B12 until active B12 is over 70

What do you mean by a 'flare-up'? Everybody means something different, so it's as well to explain.

The exact level of TPO antibodies is irrelevant. They are either negative or positive and that's all you need to know. Yours are negative. And, by the sound of it, you have had other negative results in the past. Have you ever had a positive antibody result? If not, they are of no consequence. :)

Doris11 profile image
Doris11 in reply to greygoose

I don’t think it was ever tested I will have a look 🤩

greygoose profile image
greygoose in reply to Doris11

But you said: My TPO has gone up from 9 to 11 from being diagnosed 2021! So, I assumed they were tested when you were diagnosed. If they weren't, how can you know they've gone up?

Doris11 profile image
Doris11 in reply to greygoose

Sorry I thought you ment the other one TPOA.

greygoose profile image
greygoose in reply to Doris11

Now you've got me totally confused! The other what? We're talking about antibodies, aren't we? TPOA is TPO antibodies. That's all they test. That is what is 11.7. So presumably that is what has gone up from 9. If it isn't, then what are we talking about?

Doris11 profile image
Doris11 in reply to greygoose

My apologies

greygoose profile image
greygoose in reply to Doris11

Um... I'd rather have an explanation than apologies. :) Are we talking about the same things?

Thyroglobulin

Doris11 profile image
Doris11 in reply to greygoose

Thyroglobulin Antibodies 16.40 kIU/L (Range: 0 - 115) never been tested by GP only Thyroid Peroxidase Antibodies

greygoose profile image
greygoose in reply to Doris11

OK, so I think those are unequivicably negative. :)

Doris11 profile image
Doris11 in reply to greygoose

They are phew 😮‍💨

greygoose profile image
greygoose in reply to Doris11

But you're still hypo, so doesn't make that much difference.

thyroglobulin antibodies have never been tested by my GP

Hello again :

Your T4 is only around 54% through the range and we generally best when it is in the top quadrant of it's range as this should then convert in the gut into a higher T3 reading - as it is T3 that is the active hormone which runs all our bodily functions.

It reads that your T3 is too low for you as in your own words ' brain all over the place ' -

I know on T4 monotherapy I needed my T3 at around 5.50-5.80 for mental clarity.

Your TSH is not suppressed - there's no reason not to prescribe just a 25mcg daily increase in T4 - which you could chop in half if you feel anxious about increasing too much in one go.

Antibodies wax and wane but these show you in the ranges so I wouldn't be concerned and your CRP is an inflammation marker - so your body struggling - have you other CRP readings from previous blood tests so to compare ?

Thank you will have a look as I can’t get results on NHS app but a kindly lady sent me an email of results back to 2021 when diagnosed.

Don't stress about it if not easily found - it was just a thought - as some of us just carry more than others - I just wanted to put it into context for you so not to worry you unnecessarily.

Doris11 in reply to pennyannie

gosh my B12 for may 2022 was was 70.5 pmol/L (37.5 - 150) it’s now 49.2! Thanks pennyannie ive got a Dr calling in a fortnight 🙈 or had to wait 4 wks for my dr! Will request a B12 blood test called MMA! And start 12.5 mcg levo ❤️

pennyannie profile image
pennyannie in reply to Doris11

Ok then - glad you have additional T4 to hand and I see you have already been helped on the B12 and vitamin D results.

I know now I need my ferritin up at around 100 : folate 20 ; active B12 75++ ( serum B12 500++ ) and vitamin D up at around 100.

What was the cause of your diagnosis of hypothyroid ?

Doris11 profile image
Doris11

found medichecks one May 22 CRP 2.61 mg/L (0-5)

pennyannie profile image
pennyannie in reply to Doris11

Ok - so your body is now more inflamed - was your diagnosis Auto Immune ?

Doris11 profile image
Doris11 in reply to pennyannie

Hi do you mean Hashi’s?

pennyannie profile image
pennyannie in reply to Doris11

Yes - so were you diagnosed with Hashimoto's Auto Immune Disease ?

This would explain the increased CRP reading -

I read with Hashimoto's that it is important to first heal the gut and get checked out for certain food intolerances - have you been checked for pernicious anaemia ?

Ultimately I think gluten is best avoided with wheat and dairy common food stuffs that can cause inflammation

I don't have Hashimoto's but read many forum members following the research and findings of Dr Izabella Wentz who has this AI disease - thyroidpharmacist.com

Doris11 profile image
Doris11 in reply to pennyannie

No lovely no high tpo etc…. I basically was getting symptoms, hair loss, fatigue burning feet, sleeping a lot! I looked on Dr Google and asked to have my thyroid checked! I am a retired nurse and we are like terrier dogs with a rag 🙈🙈🙈😂

pennyannie profile image
pennyannie in reply to Doris11

Ok then - so what do you mean by a ' flare ' as generally on this forum it means hyper type symptoms and highT3 and T4 levels and raised antibody levels :

Some people only get diagnosed after a thyroid scan - as their antibody never rise outside of the ranges ?

You seemed concerned about your TPO antibodies having changed by a couple of digits - though still in the range - these are associated with thyroid AI health issues.

Doris11 profile image
Doris11 in reply to pennyannie

I think I ment some symptoms have “flared” (come back) tiredness, daytime sleeping 😞 back aches and pains.. I was settled on 100mcgs levo then about a month ago I literally woke up and I was in a lot of pain in my back (it had settled enough for me get to the gym) via the Physiotherapist

Sounds like symptoms of under medication - or a maybe a ' bug ' doing the rounds:

Doris11 profile image
Doris11 in reply to pennyannie

Possibly 😮 thank you anyway for all your advice 🥰

SlowDragon profile image
SlowDragonAdministrator in reply to Doris11

Both antibodies are negative for Hashimoto’s

However

20% of Hashimoto's patients never have raised antibodies

If not had thyroid ultrasound request this is done

Or test privately (£100)

healthunlocked.com/thyroidu...

Paul Robson on atrophied thyroid - especially if no TPO antibodies

paulrobinsonthyroid.com/cou...

Gluten intolerance is often a hidden issue too.

Request coeliac blood test BEFORE considering trial on strictly gluten free diet

Sounds like you are UNDER medicated…..rather than Hashimoto’s flare (which is usually a transient hyperthyroid phase as thyroid cells break down releasing excess thyroid hormones)
