Tell patient results are ok

Good afternoon ThyriodUk

I have had my results back from the GP for my thyroid as I have Graves which has been fine for a few year now, so just have thyroid bloods done when they allow them so it’s not every year, should they be done more often.

Feeling really tired and just not myself!

Is this possible thyroid related?

Serum free T4 - 10.5

Serum TSH 0.9

B12 - 334

Folate - 18.84

Vitamin D - 60

I am on HRT also.

Any advice would be gratefully received.

Please add reference ranges for each test. Ranges vary from lab to lab so are important.

Vit B (145- 914)

I am assuming that you are not on any meds at the moment?

TSH should be disregarded if you have graves as it is often not accurate once it has been suppressed for any length of time.

Free T4 (fT4) 10.5 pmol/L (7.7 - 20.6) 21.7%

Your FT4 is on the low side and we really do need to see an FT3 to see where your active hormone is at. You could ask your GP/Endo or see link for companies offering private blood tests & discount codes, some offer a blood draw service at an extra cost. thyroiduk.org/help-and-supp...

Only do private tests on a Monday or Tuesday to avoid postal delays.

Folate is good.

Vitamin D should be around 100 - 150. Buy one that includes vit K2 to help it go to your bones. Some are available in oil or you can take it with an oily meal for better absorption. Your B12 is quite low. Are you vegan or vegetarian? If not:

B12 - do you have symptoms of B12 deficiency? The reference range for B12 is very wide and cut off point too low. theb12society.com/signs-and...

If you do then you should discuss this with your doctor for further tests for Pernicious Anaemia.

If not, then start with a methyl B12 sublingual spray or lozenge for a week, then add a good B complex. Once you run out of the separate B12 just continue with the B complex.

This B complex has all the right vitamins at a not unreasonable cost for 90 days supply(also contains folate). Once B12 is good you can stop the stand alone B12 and just continue with the B complex.

It’s really helpful.

I am not on any meds, but take vit D and B12 complex daily and have done for a few years.

No to being vegan and veggie.

Have no Endo, did ask a while back if I could be referred but as my results are ok they said I didn’t need to do this.

Will look at doing private bloods ect

If you're not vegan/vegetarian and are already taking a B complex then you have an issue absorbing B12 and should definitely persue that further with your GP.

Which B complex are you taking? Brand?

Holland & Barrett specialise in cheap, low quality vitamins on the whole. That B complex is not the active/methylated form and its also low in quantity as well.

It contains 25ug of cyanocobalmin as opposed to the 400ug (or mcgs) in Thorne Basic B or equivalent. It also contains folic acid the inactive form of folate as opposed to methylfolate which is much easier for your body to use.

Despite its claims that it is 'high dose' it really isn't at all, so you do have to compare labels of vitamins to know what you are getting is genuinely a high dose.

I'd recommend binning that product (which is where it belongs) and replacing with one of the three listed here once you have investigated PA thoroughly with your GP.

Also, the Holland & Barrett uses the Pyridoxine Hydrochloride form of vitamin B6.

So I have been taking this for years and to be honest not felt any better, but it was a doctors recommendation!

What the view of B12 injections?

Thank you for your advice as I am really struggling at the moment and have most of the B12 symptoms on the list, but thought it must be something else because I was taking them.

If you have PA then really any form of tablet is not going to get the B12 where it needs to be - in your cells. You need injections if you have PA and you may well need them.

Now you have been taking a product containing B12 it will take 3 months to get that out of your system for blood results to show where you really are.

Go back to your GP an explain that supplementing B12 hasn't been successful and you still have low B12 symptoms and insist on further investigations & treatment.

Medichecks do two tests that are allegedly conclusive, MMA and homocysteine

For MMA, on their website it stipulates that you should not take B12 for two weeks prior to the test. I know this because I’m looking into getting my son tested for PA. How correct this info is, I know not but it’s what the website says

For homocysteine it doesn’t say whether one should stop taking it or not, so I imagine it means that it doesn’t matter, B12 can be continued. I would email them to be certain
