please can you share your expertise on these results, still having constant hypothyroidism symptoms, however GP has said to drop Levothyroxine to 100, I also take Liothyrine 20 I split the dose.

All bloods taken as advised on here 🙂🙏

Did you test as recommend with timings of dose ect? 

Can you arrange a private test with FT3.  You should ensure this in in range.  

On thyroxine “Reasons for Request”  (and T3) but FT3 not tested.  

T3 particularly lowers TSH. Your TSH is supressed.  Ideally, for dr it’s in range.  Or detectable above 0.04. 

FT3 not tested, FT4 in range so dr decreasing based on TSH

Say you are well on current dose & as FT4 in range no wish to lower.  Be ready to be told low TSH mean your are over replaced or other term eg hyper and will affect heart & damage bones. 

You can try to argue the research is based on low TSH when when FT4 & FT3 are over range, but you likely find it easier to say you prefer to be well & function accepting the risks then suffer intolerable symptoms.

Refuse to reduce dose while getting more testing done

Was test done early morning, last dose levothyroxine 24 hours before test …..

Day before test split T3 as 2 or 3 smaller doses…..last dose approx 8-12 hours before test

If Ft3 is not over range you’re not over medicated

Ferritin is far too low

Request GP do full iron panel test for anaemia

Or test privately via Medichecks

Are you vegetarian or vegan

Pre or post menopause

Serum ferritin level is the biochemical test, which most reliably correlates with relative total body iron stores. In all people, a serum ferritin level of less than 30 micrograms/L confirms the diagnosis of iron deficiency.

Never supplement iron without doing full iron panel test for anaemia first and retest 3-4 times a year if self supplementing. It’s possible to have low ferritin but high iron

Look at increasing iron rich foods in diet 

Eating iron rich foods like liver or liver pate once a week plus other red meat, pumpkin seeds and dark chocolate, plus daily orange juice or other vitamin C rich drink can help improve iron absorption

This is interesting because I have noticed that many patients with Hashimoto’s disease and hypothyroidism, start to feel worse when their ferritin drops below 80 and usually there is hair loss when it drops below 50.

We have received further information the lab about ferritin reference ranges. They confirm that they are sex dependent up to the age of 60, then beyond the age of 60 the reference range is the same for both sexes: 

Males 16-60: 30-400 ug/L

Female's: 16-60: 30-150

Both >60: 30-650 

The lower limit of 30 ug/L is in accordance with the updated NICE guidance and the upper limits are in accordance with guidance from the Association of Clinical Biochemists. ‘

Don’t lower your levothyroxine dose—your doctor is reacting to your TSH levels and it’s the wrong thing to be focusing on because you take T3. Insist on your FT3 being done. If that level isn’t over-range you aren’t over-medicated.
