Hi can anyone help me with these thyroid results from last week? I didn’t have any thyroid meds until the day before and I’m on both T3 and T4 - 100mcg T4 and 5mcg T3.

TSH - 0.18 - range 0.23-5.6

FT4 - 24 - range 9-28

FT3 - 4.5 - range 3.1-6.8

just wondering if these look ok or look like I’m on slightly too much thyroid hormones?

How do you feel

Day before test, ideally you would split your T3 into 2 x 2.5mcg and take last dose approx 8-12 hours before test

Leaving 24 hours between last dose T3 and test gives false low result

But 5mcg is low dose

Is T3 prescribed

Perhaps Request addition of another 5mcg dose mid afternoon

When were vitamin D, folate, ferritin and B12 last tested

What vitamin supplements are you currently taking

Hi I’ve tried higher doses of T3 before but didn’t suit me I had palps. I’m with an Endocrinologist, who prescribes the t3 I’m actually wondering if it’s doing much at all for me at such a small dose and not sure what the results reflect.

If I took that small dose of T3 away would I need to add more T4?

FT3 might be too low. Do you feel well?

What should FT4 ideally be? I’m tired often in the afternoons at the moment

Ideal FT4 is Where you feel well in range.

Most on feel well with FT4 in top 3rd of range & FT3 at least half way.

Once on T3 the TSH tend to go low. Many say FT4 can be low and does not affect them provided the FT3 is decent. Other need both to be good In range.

Your FT3 being quite low might be causing symtoms.

Conversion of T4 : T3 is less of a factor as you take a separate T3.

Was T3 introduced as you had poor conversation?

Low folate, ferritin, B12 and vitamin D can cause symptoms which mimic hypothyroid so should also be tested.

My Endo brought in some T3 as she thought I wasn’t converting well as I was very sluggish on T4 alone however I know this is a very minuscule dose of T3 I am on, so I am wondering if it’s actually doing much at all. So it looks like I may need to get FT3 in a better range but how can I do that do you think?

Does it also look like I need more T3? The reason I’m on a low dose is because higher doses gave me palps so I suited lower doses best but unsure if it’s helping me, with those results or what I need to do 😩

You need OPTIMAL vitamin levels

When were vitamin levels last tested

Very recently they are all looking optimal I had Vit D, which I take supplements for done, Folate, B12 and iron and ferritin. Ferritin was high, B12 I’m on injections for too

Some find unless nutrients optimal tolerating T3 difficult.

Deficient nutrient can often be the cause of poor conversion? Was that investigated?

What level of dose was your increase. Can it be a smaller increment?

Your FT4 may be too high for you, you might feel better with slightly lower FT4 and higher FT3. To better balance levels.

To have slightly lower T4 what would that entail - a lower dose of T4? Or removal of the small dose of T3? My iron has always been up and down due to various auto immune illnesses. If my Endo saw these results she would say they are ok, so she isn’t much help.

I know only 1 alteration should be made at a time ie reduce T4 check levels then increase T3.

As your FT4 isn’t over range I think I look at increasing T3 1st. Then after 6 weeks of unchanged dose check levels before deciding next adjustment.

This is what I’ve read - I’ve no personal experience, as I take an antithyroid.

Levo should be adjusted by maximum of 25mcg, sometimes less. Eg split a pill in half of take every other day. I can advise on T3 doses.

Hi just to check my FT4 is 24 and range is up to 28 does that not mean it is over the range?

No it would have to be over 28 to be over range. 24 is within range just below 80% of range (9-24) so higher level in range.
