How do you feel

Was test done early morning, ideally before 9am and last dose levothyroxine 24 hours before test

Please add ranges on Ft4

How much levothyroxine are you currently taking

What brand levothyroxine and is it always same brand at each prescription

When were vitamin D, folate, ferritin and B12 levels last tested

What vitamin supplements are you currently taking

Obviously just testing TSH and Ft4 is completely inadequate

Yes took test 24 after last dose. In morning . Empty stomach etc. They only tested those two things. Iam on 150 levothyroxine.

What’s the range on Ft4 (figures in brackets after result)

GP will want to reduce dose levothyroxine

Suggest you say you want vitamin levels tested first

And that you will test TSH, Ft4 and Ft3 together via NHS private test service - Monitor My Health

If Ft3 is not over range you are not over medicated

Do Monitor My Health test BEFORE changing dose

Only do private testing early Monday or Tuesday morning.

What vitamin supplements are you currently taking

Looking at previous posts you had really terrible ferritin a year ago

All four vitamins need retesting before even considering wether you would reduce dose levothyroxine

Optimal vitamin levels are

Vitamin D at least around 80nmol minimum and around 100nmol maybe better

Serum B12 at least over 500

Folate and ferritin at least half way through range

Hi ya , yeh that's what I'm thinking there going to say reduce . I have decided to stop taking sertraline. Last couple of months been weaning down. Fully off them last 2 weeks. I think this is making the levothyroxine work better. I do feel more myself . The results are the only thing I know. But I did look up my previous back in Jan. T4 was 16.7 (11.0_26.0) which if the () numbers same this time . I'm high end at 23.1Where as tsh previously 4.50 (0.27_4.2) above range before. If () same now 0.02 . Below now

Get vitamins tested via GP as next step BEFORE agreeing to reduce Levothyroxine

And get TSH, Ft4 and FT3 tested via monitor My Health

Or agree to SMALL dose reduction

137.5mcg 4 days and 150mcg 3 days week

What if I say I feel alot better and I need to tell them I've got my self of sertraline. Change dose if feel worse?

I'm hoping or think I will ring a speak to the doctor who knows me best . Not getting fob off by a random who doesn't know me

But you do need vitamins retested

And you will need to test TSH, Ft4 and FT3 yourself via Monitor My Health

Or via Medichecks if GP won’t test vitamin levels

I think sertraline does decrease the amount of thyroid hormones in your body (result of quick google search), so you may find a small decrease in Levothyroxine works for you. However it will probably take months or even years for your TSH to recover to a level where your doctor doesn't panic. Don't let the doctor drop your dose like a stone or you will end up feeling terrible. Record all your results and if your TSH is still very low next time you have a test, point out that the trend is upwards so you don't need another reduction. Doctors can be very quick to make large reductions in Levothyroxine without having any thought about the consequences for their patients, and tend to look at isolated blood test results rather than trends over the longer term.
