advice on low T3/T4

I take T3 5mcg x3 (10mcg in day 5mcg at night)

T4 75/100mcg alternate days

so picture is latest bloods T3 down slightly from 4.4

T4 down from 15.5

TSH remains same

Feel I need to up my levels but not sure what to ask Endo to do, increase T3 and T4? Advice on way forward would be most helpful.

When did you take last dose of Levo and T3 before this test? Last dose of Levo should be 24 hours before and last dose of T3 should be 8-12 hours before.

It's not advisable to increase both at the same time, you should increase one, wait 6-8 weeks then retest and assess how you feel before deciding on next step.

Increasing Levo will increase FT4 level and it will also increase FT3 level to some extent depending on how much natural conversion you have.

Increasing T3 will increase your FT3 level and may slightly reduce FT4 level.

TSH will always be low or suppressed when taking T3, it's just what it does.

If you are discussing resuts with endo it woud be better to get a new test as two month old results may not be appropriate now.

Hi,Yes I follow guidelines on taking meds before blood tests.

Ok, so new bloods but would you increase FT 4 first then to possibly back up to 100mcg daily?

FT4: 14.6 (12-22) = 24.6% through range

FT3: 4.2 (3.1-6.8) = 29.73% through range

If those were my results then I would be increasing Levo first to 100mcg daily.

Caveat

We are all different as to where we need the individual hormone levels when on combination hormone replacement. Some are fine with low FT4 as long as FT3 is in upper range. Some need both FT4 and FT3 over half way through range.

Personally I am best around 60-70% for both (I take Levo plus T3) and your results would have me bedbound even if FT3 was raised, I have to have a higher FT4, but that is right for me, what is right for you is something that needs you to experiment, tweak and find out and that can take time so much patience and gradual tweaking is needed.
