Above 2 would be positive, & that’s within the expected limit so not Graves but you have very high TPO so there is an autoimmune issue. Your immune system is attacking thyroid.
As first the thyroid is damaged the cell release the hormone stores causing high levels, ultimately the destruction result in less functioning thyroid and under active levels.
Your ongoing hyper levels is caused by a toxic nodular goitre the nodules will continuously over produce and whereas Graves has the potential to go into remission it’s unlikely the nodules will ever remit.
Your doctor would ague for RAI or surgery now as medication won’t ultimately resolve the issue.
You also have a number of factors with thyroid - Having hyper nodules, non functioning areas an enlarged thyroid you do have symptoms & your levels are not under complete control yet.
That’s why the doctors are pushing for treatment, but you need to be comfortable with your decision.
Might be worth investigating availability and if you are a candidate for Radio frequency Ablation (RFA).
RFA of thyroid nodules is not a widely offered procedure. EG in UK only 2 National Health Service hospitals have the procedure available, neither near me sadly & I don’t have means to arrange travel.
It’s less invasive. You’d likely need assessment, 2 biopsies confirming benign nodules. The procedure itself involves local anaesthetic, ultrasound guided percutaneous needles which deliver radio waves to nodule. The body absorbs the ablated cells. RFA is better at preserving the remaining healthy thyroid function.
