New blood test - Confused no longer have low T3 - unmedicated.

Hi last post I had low T3 I have done another test with antibodies and it has all come back in range which is great thankfully, but why was it low last blood test ??

These are last weeks test came back today . No medication, raven before 9am

Thyroid Hormones

TSH 2.88 mU/L (Range: 0.27 - 4.2)

Free T3 4.01 pmol/L (Range: 3.1 - 6.8)

Free Thyroxine 14.2 pmol/L (Range: 12 - 22)

Autoimmunity

Thyroglobulin Antibodies 31 IU/mL (Range: < 115)

Thyroid Peroxidase Antibodies 13 IU/mL (Range: < 34)

Your T3 is still pretty low. And your T4 isn't very high. Your TSH, on the other hand, is too high, and shows your thyroid is struggling.

Having antibodies in-range doesn't prove you don't have Hashi's. Antibodies fluctuate all the time. And, 20% of Hashi's people never have high antibodies, anyway. So, you could still have it.

Hi GG thank you , last bld test just the T3 was low out of range and thought maybe I had Central Hypo ,

I have the symptoms of thyroid concerns but no idea which . I have now had lots of blood test take with Endo all of the ones above and way more thankfully , im just waiting to hear back from them but thought I'd have my own tests in the meantime.

Does it look like I might have Hashi's ??

You really can't tell from those results if you have Hashi's or not. You either have to have over-range antibodies, or a suspicious ultrasound, or blood-test evidence that you swing from hypo to hyper and back again.

And, at this point, there's so suggestion of Central Hypo, Which would be low TSH and low Frees. I don't think your Frees are low enough to suggest CH. Maybe they are on another test?

But, do you always test early morning, when the TSH is at its highest?

The first test was around lunchtime fasting that had low T3 , the one that Ive posted today, the second test was drawn at 7.30 in the morning before breakfast .

These were my last bloods done 2 weeks ago.

THYROID STIMULATING HORMONE

1.73 mu/L

TSH levels normal (normal range 0.27 - 4.2 mU/L)

THYROXINE

11.3 pmol/L

FT4 levels low (normal range 12 - 22 pmol/L)

TRIODOTHYRONINE

4.7 pmol/L

FT3 levels normal (normal range 3.1 - 6.8 pmol/L)

You cannot compare blood test results unless you do them the same every time. TSH is highest around midnight, and drops through the morning to its lowest point around midday. So a blood draw at lunchtime and a blood draw at 7.30 would not give the same results.

We always advise people to have the blood draw before 9 am, after fasting over-night.

FT4 and FT3 also vary throughout the day but not quite to the extent of the TSH.

Oh I see thank you the next test I have I will ensure I take it at exactly the same time ⏲️ I plan to do them again in 2 weeks. Maybe by then I will have the results back from the endo
