Attached are my latest Thyriod test.
I take 2x Thyriod S 60mg tablets a day.
Is the high TSH 20.50 mL/U/L something to be worried about ?

Those are hypo results and suggest that maybe your Thyroid-S isn't working.

Feeling generally ok, but do have stomach issues.
I did not take on Thyroid-S 48 hours before i done the test.
I was originally diagnosed with an underactive Thryiod couple of years ago.
I am getting an endoscopy soon, but wanted to check my TSH levels to see if it could be related to my stomach issues.
Would i need to take more Thryiod-S or maybe switch to something else ?

I did not take on Thyroid-S 48 hours before i done the test.
So you have false low FT4 /FT3 and false high TSH results.
Last dose of NDT should be 8-12 hours before the test, splitting dose the day before and adjusting time if necessary.
I would retest following the advice about timing to get a true measure of all your levels before doing a thing else.
It's something to be suspicious about. Because it doesn't really correspond to your thyroid hormone levels. Were these private labs? Or ordered by your doctor?

I got them ordered from a my health insurance, so yes private lab. Could i be undermedicated ?

I would say it's pretty certain that you are under-medicated, but I don't say that on the grounds of your high TSH. That is too high to be true. So, it's not just your FT4 that needs retesting, it's also your TSH. BUT, in a different lab. There could be something interferring with the results.
Should i increase to 3x Thryiod-S a day and het retested in a few weeks ?
You certainly shouldn't ever increase by 60 mg at a time. Increase by 1/4 tablet - 15 mg - and retest in six weeks.
But, quite honestly, if it were me, I would want to retest before increasing, just to be sure those numbers are correct.
