Private blood test. Please help

I’ve upped my dose too 100mg of Levo 4 weeks ago, and these are the results:

TFT’s

TSH- 21.84 (0.27-4.2)

FT4-23.6 (11-26)

T3-3.7 (3.9-6.8)

I actually feel like death. Brain fog, disconnected from reality, anxiety, depression, muscle aches, bad headaches, extreme exhaustion, feeling sick. Someone please explain to me what these results mean… I’m so confused because I need more t3, but if I add more Levo then my T4 will be out of range. And I already feel a bit hypo

Your previous results on 75mcg Levo were :

TSH 74.3

fT4 11.9 [11-25]

So after 4 week s on 100mcg we can see your fT4 has increased quite a bit (assuming you took the levo the same amount of time before both tests?)

The TSH will take longer than 4 weeks to fully respond to the increased dose .. , but it has already moved in the right direction (lower ), it may be lower after another couple of weeks.

But the fT3 is below range ,, which probably explains how rubbish you feel .

Thank you. But surely my t3 needs extra meds? But if I up my dose, my T4 will be out of range ( too high).

These results do seem to suggest you may need to add some T3 to Levo ... BUT.. these results may (all) still change significantly over the next few weeks. So you need to wait for this dose to settle in for a little bit longer before you can tell whether you have room to add more Levo without taking fT4 over range,

You TSH has been VERY high not that long ago .. and because TSH is much slower to move than T4/3 when we alter dose ,this can make it hard to interpret what's going on .

So you should probably wait for a test in another couple of weeks .. and compare it to this one, before deciding about whether you may need T3 added or whether you simply need an increase of Levo .

Thank you very much for your answer. I have an appointment with my endo son and I think he might suggest NDT
