Hello , does anyone have any knowledge of block and replace. I currently take 100mg of Leveo on an empty stomach when I wake up and them 40mg of Carbimazole at least an hour later. Is this the best way to space them or is it better to leave more time? Take Carbimazole later? I am not sure of the best combination

Sorry just seen your post.I'm currently on my second lot of block and replace.I take the same doses as you .I take my levo in the morning on an empty stomach and wait 1 hour before breakfast but I take my carbimazole at night before going to bed.Ive found this works for me.

Hello, I was diagnosed with graves in Sept 2018.I was started on 40 mg of carbimazole.My ft4 was 57.1 range 12.0 to 22.0. My ft3 was 23.1 range 3.1 to 6.8. The end o tried titration until Dec 2019 but due to hospital appointments being too far apart my levels kept going too high then too low. He decided to try block and replace Dec 2019 my ft4 was 10.0 my tsh was 7.75 at this stage, I have no ft3 reading.I stayed on B and R until Sept 2020 and he decided to take me off my tablets as I was euthyroid( his words).I felt as if my hyperthyroidism had come back so I had a blood test on 13 Oct 2020 and my ft4 was 69.6 no ft3 reading.I started on 40mg carbimazole straight away and have worked my way back up to 100mcg levothyroxine. I am to have a blood test and will be reviewed in 3 months.I haven't had my antibodies tested since the beginning as he said it wasn't necessary. In August 2018 they were TSH receptor Ab (OR)25.7 IU/L 0-0.9 . Imm Thyroid Peroxidase Ab 6 IU/mL 0-34. Hope I've written this right, not very good with computers or phones.Just copied from hospital letter.Hope this helps.
Thank you for the reply. I deeply appreciate that. I’m really struggling. I couldn’t get on with the carbimazole so I went to PTU. I like this drug. I am struggling with T4 being bottom of the range at 11 range 10-24 and my T3 is still over at 12.3 range 2.0-6.0. I have felt suicidal on these levels (levels would have been lower as I continued the ATD) and I am thinking would block and replace be an option. How did u feel when your levels were low?
Sorry to hear about your experience.When my levels were low I felt tired and had very achy joints as if I had flu.My skin and hair was very dry.My hair was horrible really, coming out in clumps when I washed it.My eyes were also dry, gritty and bloodshot.I do have thyroid eye disease and it definitely made it worse.Regarding my mood I amnormally an optimistic person but I can say I did feel fed up at times .A word we use where I'm from is morngy. I had to just get away from everyone , maybe go and lie on my bed for a while.I also felt more anxious and couldn't cope with any stress.I still feel it now with my graves but it was worse. I read that carbimazole can cause achy joints but so can graves.I do take 20mg amitriptyline each night for migraine prevention and that is also used for nerve pain so that may be helping me with that .Could it be going hypothyroid that is effecting you badly rather than the carbimazole itself? I was left too long between appointments so I kept yoyoing up and down.I wish sometimes I could just have my bloods done, get the results and do what needs to be done dose wise straight away but of course that would be too easy.I don't really know about put, is it the same as carbimazole? Can you use it on it's own or with levothyroxine for block and replace.?I'm not very knowlegable really.I don't understand why your ft4 is low but your ft3 is high.You may be better starting a new question and seeing if any of the more knowlegable members can help. I'm so sorry you are going through this but someone may be able to give you better answers than me.Best wishes to you.
Your answers have been helpful. Carbimazole made me physically sick. PTU no physics side effects. I have a good dr he does blood every 4 weeks. He’s young and I feel not experienced with this. He had never heard of PTU. When Un medicated T4 was 100 range 10-24 and my T3 was 48.5 range 2.0-6.0. The thyroid drugs abliterate the T4 but T3 stays over range. I flip flop on my drugs because I feel mentally unwell. Thank u for your help
Sorry I couldn't be more helpful because it does sound like you are having a very hard time.As I said maybe someone else could work out why ft4 is low but ft3 is high.Alot of people on here seem to have good knowledge and might have the key to solving your problem.My thoughts are with you.
