New blood results after taking Carbimazole 5mg

Please could I get some advice. My new results since taking Carbimazole 5mg for 8 weeks are TSH 0.04 —T4 is 14 (was 21.1)— and free T3 is 5 (almost same as before) Only the TSH is out of the range now as it has been for some time. All Autobodies test I believe have come back negative. I need to confirm which ones I have had. Not gluten intolerant either but low on B12, Folate 7.4ug/L. >3.0. Will find out what my Iron levels are. MCV 106 fL (80-100)

21.1 is a low ft4 result for hyperthyroidism. I would check exactly what antibodies were tested and the results. For hyperthyroidism you need to check TSI and TRAB antibodies.

Autoimmune thyroid disease aka hashimotos aka hypothyroidism often starts with a hyper phase. The results of this hyper phase can be misinterpreted as true hyperthyroidism.

Had a look at your previous posts.

Can you clarify if this the progression of your results?

16 AUG 2019 EDIT(corrected from Nov)

TSH 0.21 (0.35-5.50).

T4 14.4 pmol/L (7.0-17.0).

Why were these 2 taken so close together? (N/a 3m apart)

26 Nov 20 2019

TSH 0.28 (0.35-5.50).

T4 13.2pmol/L (7.0-17.0).

FT3 5.1 (3.5-6.5)

26/11/2020

TSH: <0.02 (3.5-5.5)

FT4: 21.1 (7-17) (141%)

10/12/20

TPO antibodies: 0.2 (<9.0)

I understand from these results you were put on 40mg carbimazole but this was reduced a few days to 5mg per day. (Which is a very low dose)

8 weeks treatment

Recent? End jan 21

TSH 0.04 — (0.35-5.50).

T4 is 14 - (7-17) (70%)

T3 is 5 - (3.5-6.5) range?*supplied

So being on your antithyroid has reduced your FT4 from being over range to 70% in range. I can’t find previous FT3 result so we don’t know how this been altered?

Do you currently feel well?

Has your doctor told you what is causing your hyper?

Are these results taken from a printed blood test? Or from an online access system?

The first should be 16 August 2019. T3 on 26 Nov 2020 was 5.1 (3.5-6.5) I currently feel Ok but my symptoms that Ive had for some years now come back. Hence the repeated blood tests.

Ok so there’s been .1 change on FT3. Have you been advised to stay on same dose of carbimazole, or alter it? When did your symptom go and reappear?

The results are printed and I do not know the reason for my results. You have the correct blood results apart from the date of the first- see previous reply.

Just wanted the double check correct range. If on paperwork, you are correct. The FT4 of 21, when I first glanced thought would probably be in range. Your range seems lower. Some apps system allow practice users to set different limits to the lab, so that’s why I asked.

Yes the range for T4 on the paper is(7.0-17.0)

Leaving aside the thyroid stuff, your MCV is overrange. Which means you have big blood cells—and is a result of having very low B12 and/or folate.

Just how low are those results? Because I think some of the unwellness you’re experiencing is more likely to be due to your low Vit B12/folate. It might have very little to do with what your thyroid hormone levels are (they’re looking quite decent actually - don’t worry too much about the TSH as it can take quite a while for it to come back up when a hyper spell has dampened it down. It doesn’t mean you’re still hyperthyroid).

Yes Ive been asking them about B12 as it is too low and was getting worse. Its just above the lowest limit though so they dont worry about it. Its very frustrating. I have started taking high dose sublingual tablets to try and help but think that I should take VitB injections to make sure my body is getting enough?

Where should levels of folate be?

I’m not an expert but would aim for a number nearer 20?
