Normal TPO and high TgAb and pregnancy

Hey everyone!

What does that mean when your TPO is within range but your TgAb are seriously elevated?

I have just found out I am pregnant and am still waiting on my thyroid panel from the GP but also went for the Medichecks full thyroid check. I know what to do with my levothyroxine but the antibodies really worry me. I’m not my usual calm self and feel very anxious.

TgAb 408 kIU/L [<115]

TPO 15.2kIU/L [<34]

TSH 3.06 mIU/L [0.27 - 4.2]

Once I have calmed down, I’ll create another post with all my results. I never panicked like that with my first pregnancy. I don’t know what came over me. Thank you for any insight you may be able to share.

Because your TSH is high, that will increase TG antibodies

Seems to be confusion as to wether you have Graves’ disease or Hashimoto’s

Have you had TSI or Trab antibodies tested to confirm Graves’ disease

Have you increased your levothyroxine?

Essential to test vitamin D, folate, ferritin and B12

What vitamin supplements are you currently taking

Are you on strictly gluten free diet?

Thank you so much SlowDragon!

Yes, I definitely have hashi and not Graves. My endo had my TRAb tested in August last year after a hyper swing.

As per my endo’s advice, I increased my dose by 25mcg (I was at 50mcg). The raise in TSH must be quite recent because I have been feeling so well until only a few weeks ago!

Here are my other levels. Apart from my folate and B12, they don’t look very good. My vit d is shocking, and that’s with me taking 3000IU of D3 and K2 a day for the past 2 weeks (after reading an article on covid and vit d levels). I also take 400mcg of L-methylfolate a day.

I’m also extremely anxious. I wonder if it’s normal or if I’m just being over dramatic. It’s my first pregnancy since my diagnosis and I managed to terrify myself with googling.

I’d also recommend reading up on these articles. According to Dr Izabella Wentz, known as The Thyroid Pharmacist, your TSH should be no more than 2.

These two articles by her are about the thyroid and pregnancy and are invaluable:

Thank you very much for those links! I’m going to take the time to read them properly. My levels were so good throughout the autumn, I felt full of beans in November, and even first half of December... Now that I’ve calmed down a bit, I’m thinking it’s my diet over Christmas that’s made things worse for me...

I think when you are first pregnant, you need higher FT4 levels as the baby is using up some of yours (may not quite be how it is, but I’m not a doc).So it makes sense to me that you need to increase your thyroxine dose at the moment. It may be that you reduce the dose again after the birth, but again, I hope your GP will know all about that.

Thank you for your reply, Lovecake. I haven’t had the chance to discuss with my endo yet as I received my results too late on Friday. But she did tell me to immediately increase my meds by 25mcg should my thyroid be over 2.5. She will hopefully get back to me on Monday to confirm the increase or increase it more.

I feel a big ashamed at how panicky I was last night...

Don’t feel ashamed, I would say you are being normal. Am sure you will feel ok soon and be able to enjoy your pregnancy, congratulations by the way ☺️

Hi I thought that I would try to help if I can by saying once I was diagnosed with antibodies my thyroid problem changed!! I was not prepared for my problems turning to RA which is an autoimmune disease, which has now causing me to have Sjogrens disease! also autoimmune disease!! So I wanted to say ask your ENDO to keep a close eye on your situation , I wish you all the best HungryHypo.Regards wakeham
