Thank you to those who responded to my last post. You all have provided great advice and information and I am grateful.

I just wanted to give you an update on what happened when I spoke with my doctor... it’s safe to say the doctor was not happy I got a private blood test done, she was confused to the results being so different and was basically slagging off the results saying vitamin d should be tested as well as calcium and some other stuff. When we discussed my thyroid results I asked how they can be so different and said the only difference is I took my blood tests at different times of the day. My doctor point blank told me that she has never heard of having to take the thyroid blood test in morning and not taking medication until after and said this shouldn’t make a difference!!

Basically she has requested a re test for my thyroid, vitamin d, b12 etc. And then we will go from there with the results.

I didn’t even get a chance to ask about coeliac disease so I will next time or just get one privately.

I feel like my GP has no idea how to treat me, if my results come back abnormal I think she is going to refer me to a specialist.

I am so glad I got a private test done as they would have considered me ‘normal’ and that would be that.

I think it is good they've sent off for a re test, mine wont even test for vitamin D or B12 so I think you have a good doc despite the issues!

My consultant did say it does not matter what time of day the thyroid test is taken, and so I think sometimes you do get conflicting advice on here, I personally do book my test for the morning however I sometimes wonder where that info comes from as it takes years to train to be a consultant surely they would say take in the morning if it matters? I guess they normally write time test taken on the test so maybe they take into account

The fact they are saying they will refer you if they come back abnormal again - well at least it shows they are investigating and doing their job?

They wouldn’t have unless I got a private test and it came back insufficient. I know it’s bizarre however I did my private test in the morning and it gave a completely different test to when I had mine done in the afternoon. They were like a couple of weeks apart and no change in medication. It will be interested to see what my retest comes back as but from now on I will continue morning testing etc.

Oh right but it can take time for meds to work so maybe just seeing results from the meds? I guess when you get the next results you will know more I agree it is best to stick to morning to keep that factor constant

My consultant did say it does not matter what time of day the thyroid test is taken

There is research evidence available to say he's wrong and it is discussed in this old thread.

Previous post with TSH test results shows that you needed dose increase

What did GP say about severely deficient vitamin D

You should be prescribed LOADING dose vitamin D at 300,000iu over next 6-8 weeks

Print these guidelines out and give them to GP

Median TSH graph

TSH daily variation

Extremely important when you test TSH ....

researchgate.net/publicatio...

According to the current TSH reference interval, hypothyroidism was not diagnosed in about 50% of the cases in the afternoon.”

“Further analysis demonstrated inadequate compensation of hypothyroidism, which was defined in 45.5% of the morning samples and in 9% of the afternoon samples”

ncbi.nlm.nih.gov/pubmed/252...

TSH levels showed a statistically significant decline postprandially in comparison to fasting values. This may have clinical implications in the diagnosis and management of hypothyroidism, especially SCH.

NHS England Liothyronine guidelines July 2019 clearly state on page 13 that TSH should be between 0.4-1.5 when treated with just Levothyroxine

Note that it says test should be in morning BEFORE taking Levo thyroxine

Also to test vitamin D, folate, B12 and ferritin

See the Doctor wasn’t going to increase my medication with the results they had. They class that as normal..nothing was prescribed until receive the re test results.

Despite all guidelines saying that when on levothyroxine TSH should be under 2

You likely will need a recommended endocrinologist....GP beyond hopeless

Come back with new post once you get NHS test results

B12 is EXTREMELY low

GP should be apologising for not testing......rather than kicking off that you got private testing

Exactly! Well I will just wait to hear what they say next. I’m thinking of changing surgeries anyway as I’ve moved so hoping I get a doctor that understand thyroid problems better.

It would be hard to be more useless than current one
